For elderly people and people with disabilities who find it difficult to get to medical institutions, etc., where vaccinations are given because they use wheelchairs when going out on a daily basis, taxis with lifts that can be used in wheelchairs or stretchers We will support transportation to the vaccination site.
The city will cover up to 10,000 yen per ride (round trip from your home to the vaccination site), including reservation fees, pick-up fees, fares, and additional fees such as assistance fees.
* If the amount exceeds 10,000 yen, the excess amount will be borne by the user.

Target audience

Those who live in Nerima Ward and fall under any of the following (A companion may ride with you.)
1. Those who are certified as needing long-term care 3 to 5 and use a wheelchair or stretcher when going out on a daily basis.
2. Those who have a physical disability certificate, love certificate, or mental disability certificate and use a wheelchair or stretcher when going out on a daily basis.
3. Those who do not fall under any of the above, but who have difficulty moving to the vaccination venue, such as using a wheelchair on a daily basis.

Usage flow

1. After completing the vaccination reservation, call the taxi operator that has a contract with Nerima Ward from two weeks to the day before the scheduled date of use (see "List of contracted operators" below) and ask, "For the purpose of vaccination Inform them of the following 8 items and make a request.
(1) Date and time of use (2) Name (3) Address (4) Telephone number (5) Level of care required/presence or absence of a notebook (6) Regular use of a wheelchair, etc. (7) Presence or absence of an attendant (8) Nerima vaccine Vaccination ticket number (coupon number)

2. On the day of vaccination, we will pick you up from your home. *Please prepare the "vaccination ticket", "pre-examination slip", and "identity verification documents" necessary for vaccination in advance.

3. After getting on the bus, show the driver your vaccination ticket, nursing care insurance card, or handicapped person's handbook . If you do not have an insurance card or notebook, you will be asked to sign a prescribed application form stating that it will be difficult for you to move to the vaccination venue.
