Those eligible for reimbursement
Those who meet all of the following requirements
- Women born between April 2, 1997 and April 1, 2005
- Registered as a resident in Meguro Ward as of April 1, 2022
- Inoculated at own expense before March 31, 2022 (inoculated other than routine vaccination or catch-up vaccination)
- The type of vaccine inoculated is bivalent (Cervarix) or quadrivalent (Gardasil) (9valent (Sylgard) is not eligible)
- Inoculated at a medical institution in Japan
Amount of subsidy for expenses
The lower amount will be subsidized between the actual inoculation cost and the maximum amount set by the ward.
Also, please note that document preparation fees are not covered by the subsidy.
Application method and grant flow
1. How to apply
In order to use the subsidy system, an application is required.
Please submit the "application documents" and "attached documents" by mail to the Health Prevention Section Vaccination Section or at the counter.
2. Application documents
The application documents to be submitted differ depending on the presence or absence of attached documents, so please check the sample entry that matches your attached documents in the "Quick Reference for Filling in Application Forms" in " 4. How to Apply for Expense Subsidies ".
Please note that you cannot make corrections with a correction mark or double lines on the "Application for Reimbursement of Voluntary Vaccinations for Human Papillomavirus Infection", so if you make a mistake, please rewrite it from the beginning .
3. Attachments (common)
- A copy of a document that can confirm the name, address and date of birth of the person to be vaccinated <br>Resident card, driver's license, health insurance card (both sides), etc.
- If the names of the applicant and the recipient are different due to marriage, etc., documents that can prove that they are the same person , passport, etc.

4.How to apply for the cost subsidy
Please submit the following documents to the Immunization Section of the Health Prevention Division by mail or at the counter.

Attachments to submit
- When submitting a maternal and child health handbook, please submit two pages of the "certificate of birth notification" and "page of the relevant vaccination".
- When submitting a receipt, etc., please be sure to submit the original.
- Additional documents may be requested if the applicant and the recipient are different, or if the required documents are insufficient.
4.Payment of inoculation fee
We will examine the submitted "Voluntary Vaccination Reimbursement Application Form for Human Papillomavirus Infection" and attached documents, and send a notification of payment decision for the cost of vaccination.
The vaccination fee will be transferred to the bank account specified on the application form, but we will not notify you of the payment.
Please note that it takes about 1 to 2 months from application to payment.
