About child medical expenses subsidy system
This is a system of the ward that aims to support the improvement of children's health and healthy development by subsidizing part of the medical expenses related to children for those who are raising children.
Target audience
People who live in the city, are enrolled in health insurance, and are raising children up until the end of the third year of junior high school (the first March 31st after the child turns 15).
* From April 2023, it will be expanded to the age equivalent to the third year of high school (the first March 31st after reaching the age of 18).
Infant Medical Certificate (Maru Milk)
Preschoolers (until the first March 31 after reaching the age of 6)
Child Medical Certificate (Maruko)
Elementary and junior high school students (until the first March 31 after reaching the age of 15)
High school student medical certificate (circle blue)
High school age equivalent (until the first March 31 after reaching the age of 18)
You are eligible even if you are not in high school.
* High school student medical certificates will start from April 2023.
Those who are currently in the third year of high school are not eligible.

- Children in facilities where medical expenses are covered by public funds (child welfare facilities)

- Children placed in foster care
Scope of grant
What can be subsidized
Self-pay portion due to health insurance coverage when receiving medical treatment or dispensing using health insurance (20% or 30% of total medical expenses covered by insurance)
What cannot be subsidized
- Dietary treatment standard burden when hospitalized (Note 1)
- Items not covered by health insurance (Note 2) (medicine container charges, hospital bed charges, medical examination charges, vaccination charges, medical certificate charges, etc.)
- Part of high-cost medical expenses and additional benefits paid by health insurance (Note 3)
- When the Japan Sport Council Act is applied (Note 4)
- Applicable parts of other publicly funded medical care systems (Note 5)
- In the case of medical treatment by a third party such as a traffic accident (Note 6)

In the case of households exempted from resident tax, the standard cost of meals for hospitalization may be reduced. Please contact your health insurance association, etc.

One-month-old child medical examination is out of the object of the furtherance. Health checkups for infants notified by public health centers are free of charge.

Whether or not there is a system for additional benefits and the standard amount for payment will differ depending on the health insurance you are enrolled in. (The Katsushika City National Health Insurance and the health insurance managed by the Japan Health Insurance Association do not have an additional benefit system.)
If you are hospitalized, please receive a ceiling application certificate from your health insurance and present it to the medical institution along with your medical certificate. (This is not necessary if the electronic eligibility such as My Number can be confirmed on the health insurance card.)

If you are a member of the Japan Sports Council Disaster Mutual Aid System and the total medical expenses (100% covered by medical insurance) from the initial consultation to recovery are 5,000 yen or more when you are injured at school or nursery school, Condolence money, etc. will be paid. You cannot receive the subsidy for children's medical expenses at the same time.

If you are eligible to use other public medical expense subsidy programs, you will need to apply for each. Please contact us as we may be able to subsidize the co-payment after using other systems. In Katsushika Ward, please give priority to medical certificates for infants, children, high school students, etc., only for mentally and physically disabled medical certificates (circular disability) and single-parent household medical certificates (circular parents).

In the event of injury due to a third party's actions, such as a traffic accident, medical expenses, etc., are originally borne by the assailant. Health insurance permits the use of a health insurance card on the condition that the right to claim damages against the assailant is subrogated. If the use of health insurance is approved, you can use the medical certificate for infants, children, high school students, etc. after the same procedure, so please be sure to contact us.
To receive a medical certificate
An application is required to receive a medical certificate.
The medical certificate qualification date is the date of birth for those who were born, and the date of moving in for those who moved in. However, if you do not apply within 3 months from the date you become eligible, the date of application will be the date of acquisition of eligibility.
Applications can be made by mail or online from the Katsushika City official website, as well as at the ward office.

High school student medical certificate (circle blue)
From April 2023, subsidies for children's medical expenses will be expanded to children of the age equivalent to high school students.
High school student medical certificate will be issued after March 2023.
If you have completed procedures to move into Katsushika City, please apply for issuance at the Child Care Support Division or the Residents' Office.
You can apply by mail or online on our website.
Those who live in Katsushika Ward and are eligible for high school medical care certificates and were born between April 2, 2005 and April 1, 2007 will be eligible for high school medical care in late December 2022. We will send you a special application form for the certificate by mail, so please prepare the necessary documents and submit it. Those born between April 2, 2007 and April 1, 2008 who are currently in their third year of junior high school and have a Katsushika Ward child medical certificate do not need to apply. The medical certificate will be issued after March 2023.

Health insurance card
- The health insurance card of the child receiving the medical certificate is required.
- In the case of a birth, it is possible to apply with the health insurance card that the child plans to join (eg, the father's health insurance card if the child is dependent on the father).
- If you apply by mail, please submit a copy of your health insurance card. For online application, please attach electronically.
Identity verification documents

Weekdays from 8:30 a.m. to 5:00 p.m. However, from 8:30 a.m. to 7:30 p.m. every Wednesday (excluding national holidays and year-end and New Year holidays). In addition, we do open agency from 9:00 a.m. to noon only on holiday open agency window (once a month Sunday).
*Reception of households including children eligible for medical certificates for high school students will be handled by the Child Care Support Division.

Weekdays from 8:30 a.m. to 5:00 p.m. However, from 8:30 a.m. to 7:00 p.m. every Wednesday (excluding national holidays and year-end and New Year holidays). In addition, each city resident office does not open on holidays (once a month on Sunday).

Issuance of medical certificate
<Households with infants and children eligible for medical care certificates only>
If you apply at the Child-rearing Support Division (Counter 401 on the 4th floor of the Ward Office) or the Family Register and Resident Section (Counter 217 on the 2nd floor of the Ward Office), the application will be issued immediately if the necessary documents are complete. If you apply at a ward office, apply by mail, or apply online, we will mail it to the resident registration address of the child listed on the medical certificate.
<Households including children eligible for medical certificates such as high school students>
Medical certificates for high school students will be mailed after March.
If you apply for an infant/children's medical certificate at the Child-rearing Support Division (counter 401 on the 4th floor of the ward office), it will be issued immediately if you have all the necessary documents. If you apply at a ward office, apply by mail, or apply online, we will mail it to the resident registration address of the child listed on the medical certificate.
For those who have submitted a birth notification outside the ward or at night/holiday counters
Please apply at the Child-rearing Support Division (Counter 401 on the 4th floor of the ward office), at each ward office, by mail, or online.
How to use the medical certificate
When visiting a doctor using a medical certificate
If you present your health insurance card and medical care certificate for infants, children, and high school students at the counter of a medical institution in Tokyo, there is no copayment. Infants, children, high school students, etc. The medical certificate can be used at most hospitals and pharmacies in Tokyo.
When you see a doctor without using your medical certificate
- When you receive a medical examination at a medical institution outside of Tokyo or at a medical institution that does not handle medical care certificates for infants, children, high school students, etc.
- If the head office of the health insurance you are enrolled in is located outside of Tokyo, the National Health Insurance Association
- When you see a doctor before receiving a medical certificate
- If you forget or lose your medical certificate
In these cases, please pay the copayment (20% or 30%) at the counter, receive a receipt, and later apply for a refund of medical expenses to Katsushika City. We will pay the co-payment after health insurance is applied. Please check the details from the following page.
Notification is necessary in such a case

- When there is a change in the name of the parent or child
- When parents change due to marriage, divorce, etc.
- When you move out of the city (If you move within the city, please write your new address on the back of your medical certificate.)

Reissuance of Medical Certificate
Please apply for reissuance by either Child-rearing Support Division (Counter 401 on the 4th floor of the Ward Office), mail application, or online application. The medical certificate will be issued immediately if the application is made at the counter. If you apply by mail or apply online, we will mail it to the child's resident registration address on the medical certificate.

Renewal of medical certificate
The medical certificate is renewed on October 1st every year. A new medical certificate will be mailed around the end of September.
*The validity period of the infant medical certificate for children who will be in the first grade of elementary school in April and the child medical certificate for children who will be in the first grade of high school is until the end of March. From April, 1st grade elementary school students will switch to a child medical certificate, and children equivalent to 1st year high school students will switch to a high school student medical certificate. A medical certificate will be sent to those eligible in late March.
