Of those who have moved in, those who fall under the following are required to apply.

• Persons aged 5 and over who wish to receive the first and second vaccinations

• Those who have not received the vaccination ticket even after 3 months have passed since the 2nd, 3rd, and 4th vaccinations

(explanatory note) We automatically send to person reaching 5 years old after two weeks after moving-in notification day.
(Note) If there is an incomplete vaccination record at the time of vaccination in the municipality before moving in, or if you were vaccinated overseas, the ward may not automatically send the vaccination ticket.

You can apply for one person with one application.
The destination of the vaccination ticket will be the location of the certificate of residence of the person to be vaccinated.

If you have moved to Nerima Ward and have already been vaccinated once or more, please attach a document that shows your vaccination history (such as a vaccination certificate) with photo data.
(explanatory note) When sending is hoped for outside resident's card address , please use mail application as there are necessary documents separately.
Please note that we cannot accept your application if any of your name, address, or date of birth is entered incorrectly.
We do not respond to inquiries regarding application acceptance.

(Note) Applications will not be accepted from 10:00 pm on Wednesday, January 25, 5th year of Reiwa to 5:00 am on Thursday, January 26th, 2023 due to scheduled maintenance.
