Chuo Ward Disabled Welfare Service Office Subsidy for Emergency Support Project for Soaring Prices

By December 1, 2022, food expenses, utility costs, and fuel costs for vehicles used for transportation and visits will be paid to the disability welfare service office in the ward designated by the Tokyo Metropolitan Government or Chuo Ward. We will provide support against rising prices.

Target service

(1) Admission services

Assistance for communal living, short-term admission

(2) Outpatient services

Life care, accommodation type independence training, independence training, employment transition support, employment continuation support type A, employment continuation support type B, employment settlement support, child development support, after school day service, home visit type child development support, nursery school, etc. Visit support

(3) Visiting services

In-home nursing care, home-visit nursing care for severely injured persons, accompanying support, behavioral support, comprehensive support for persons with severe disabilities, etc.

(4) Consultation services

Independent living support, planning consultation support, community consultation support, handicapped child consultation support

  • (Note 1) Total number of users including residents outside of Chuo Ward
  • (Note 2) If the same vehicle is used for multiple services provided within the same office, please apply for only one service.
  • (Note 3) Each service office that has been designated after November 1, 2022 will provide the amount provided in October 2022 as of October 1, 2022 as of the designated date. It shall be read as the specified date and month provided.

  • (Note 1) Total number of users, including residents outside of Chuo Ward. However, if there is a user who also provides nursing care services provided within the same establishment, the user is excluded. Please apply to the Nursing Care Insurance Section for the users who have been excluded because the Nursing Care Insurance Section has also established a similar support system.
  • (Note 2) If the same vehicle is used for multiple services provided within the same office, please apply for only one service. In addition, if the same vehicle is used for nursing care services provided within the same office, that vehicle is excluded. Please apply to the Nursing Care Insurance Section for the vehicles that have been excluded, as the Nursing Care Insurance Section has also established a similar support system.

  • (Note 1) For admission services, please submit Form 2.
  • (Note 2) Please submit Form 3 for outpatient services (other than employment settlement support, home visit-type child development support, and visit support for nursery schools, etc.).
  • (Note 3) Please submit Form 4 for outpatient services (employment settlement support, home visit type child development support, nursery school visit support).
  • (Note 4) For visiting services, please submit Form 5.
  • (Note 5) For consultation services, please submit Form 6.

(3) A copy of the service designation notice or renewal notice
(4) Operational regulations <br>Entrance and outpatient services (other than employment settlement support, home visit type child development support, daycare center visit support) only.
(5)National Health Insurance Federation billing statements provided in October 2020 (only for non-residents of Chuo Ward)
Only outpatient services (other than employment settlement support, home visit-type child development support, visit support for daycare centers, etc.), visiting services, and consultation services with users outside of Chuo City.
(6) A copy of the vehicle inspection certificate <br>Only for offices that apply for fuel cost subsidies for commuting, visiting, and consulting.
