About medical consultation room

In April 2016, the Itabashi Ward Medical Association Home Medical Center Recuperation Counseling Room (hereinafter referred to as the "Recuperation Counseling Room") started as a project of the ward.
The Medical Counseling Room is designed to provide medical and nursing care services suitable for patients and their families in response to consultations from patients and families who are transitioning to home care, and patients and families who are currently undergoing home care. , introduces and secures cooperation with the nearest medical and nursing care resources, and provides support for patient admission and discharge procedures.

Counselors include nurses with care manager qualifications and medical social workers (MSWs). Counseling is provided in cooperation with various occupations such as each medical institution, local Otoshiyori Consultation Center (Regional Comprehensive Support Center), care manager, and consultation support staff.

Anyone can consult

We accept consultations from 0-year-old babies to the elderly, regardless of age, disease name, or whereabouts. Professionals are also welcome to use it.
