・We cannot accept applications at the counter.
・If an administrative scrivener or a lawyer is entrusted by the establishment representative and applies, please attach a power of attorney and identification documents (copy of administrative scrivener certificate, etc.) that can confirm that.

Points to keep in mind when creating

1 Please submit all documents, including attached documents, at the time of reception.
We cannot accept your application if we are unable to confirm the facilities for your business, the right to use the building, and the funding for the equipment due to insufficient documentation or omissions.
For the liabilities to be handed over to the incorporated corporation, please check in advance the “Precautions for preparation” of the liability-related form in the guideline for establishing a medical corporation , and prepare well in advance of the application period. If there is a lack of documents, the debt cannot be transferred.
2 It is not permitted to change the application items after the application documents have been submitted.

4 Please submit the application documents without affixing your seal.
After the examination is completed, you will be asked to submit the stamped documents.
5. Submit a copy of the certificate of registered information and seal registration certificate.
Please submit the original at a later date.
6 Persons who are not administrative scriveners or lawyers are prohibited from creating documents and electromagnetic records to be submitted to public offices as a business (except as provided by law).
7 Once the documents have been reviewed, the person in charge will contact you.
