- Those who received the third vaccination by April 30, 2022 but have not received the vaccination ticket by August 10 (Wednesday)
- Those who work at facilities for the elderly outside Nerima Ward
- Those who do not correspond to the following item "Those who do not need to apply"
(explanatory note) It is necessary for five months or more to pass from the completion of the third inoculation to receive the fourth inoculation.
(Note) Please check the table below for the scope of workers such as medical workers and elderly facilities.
(Note) As this project is scheduled to end on September 30, 2022, those who received the third vaccination after May 1 cannot receive the fourth vaccination.
Those who do not need to apply
For those who fall under the following categories, we will send the vaccination ticket on the specified date, so there is no need to apply.
- Those who, at the time of the first vaccination, applied for issuance of a medical examination slip with a vaccination ticket as a resident of Nerima Ward and a medical worker, and received the third vaccination by April 30, 2022.
Vaccination tickets will be sent out on Thursday, August 4th. (Note) If you do not receive it by Wednesday, August 10, please apply.
- Those who work at facilities for the elderly in Nerima Ward and who have responded to a survey conducted by Nerima Ward asking for issuance of vaccination tickets
Vaccination tickets will be sent out on Friday, August 12th. (Note) If you do not receive it by Thursday, August 18, please apply.

- This project is scheduled to end on September 30, 2022. Those who received the 3rd dose after May 1st cannot receive the 4th dose with an interval of 5 months. Therefore, we cannot accept applications from those who have had their third vaccination after May 1st.
When to send the vaccination ticket
The 4th vaccination ticket for those who applied will be sent within 2-3 weeks after the application.
