Dissemination of the "Pharmacy Near-miss Case Collection and Analysis Project 27th Report" and "Pharmacy Near-miss Case Collection and Analysis Project 2021 Annual Report"

The pharmacy near-miss event collection and analysis project is a project subsidized by the Ministry of Health, Labor and Welfare by the Japan Council for Quality Health Care (hereinafter referred to as the "organization"). Collect, analyze and provide. The purpose of this project is to widely share information useful for medical safety measures with each pharmacy, provide information to the public, and further promote medical safety measures.

This time, the organization collected and analyzed the near-miss incidents reported from January 2020 to June 2022, "Pharmacy near-miss incident collection and analysis project 27th report". Also, the 2021 Annual Report of the Pharmacy Near-miss Case Collection and Analysis Project, which summarizes the contents of the collection and analysis of near-miss cases reported from January to December 2021, has been released.

Please use this report to understand the causes and trends of near-miss incidents, and take further measures to prevent them from occurring.
