Since certain scientific findings have been obtained under the Omicron strain epidemic, it was deemed appropriate to apply the obligation to make efforts for pediatric vaccination from September 6, 2020 (Tuesday). Target audience
Those aged 5 to 11 who have a resident card in the ward and wish to be vaccinated. 5 months have passed since the second vaccination for booster vaccination (3rd time).
(note) consent and companion of protector are necessary for inoculation. Please consider the merits and demerits of vaccination, and consult with the person (child) who is to be vaccinated before making a decision.
Vaccine to use
Pfizer (for children aged 5 to 11)
(Note) The pediatric vaccine is a different formulation from the Pfizer vaccine and Takeda/Moderna vaccine for those aged 12 and over, and the dosage and administration are different.
(Note) For those who received the first vaccination at the age of 11 and will be 12 years old at the time of the second vaccination, the second vaccination is also for children because the type of vaccine used for children over 12 years old is different. (Note) If a person who received the pediatric vaccine in the first vaccination and is over 12 years old at the time of the third vaccination, the vaccination will be for those aged 12 and over.

Inoculate after 3 weeks have passed since the first inoculation.
3rd inoculation
Administer after 5 months from the second vaccination.
Simultaneous vaccination with other vaccines
When receiving other vaccinations before or after child vaccination (ages 5 to 11), in principle, please take vaccinations at intervals of 13 days or more. However, simultaneous vaccination is possible only for influenza vaccine. (Note) Please pay attention to the vaccination interval between the DT vaccine and Japanese encephalitis second stage vaccination. If you have any concerns, please consult your family doctor.
About extension of the expiration date of the new corona vaccine
The expiration date of a vaccine is set based on data collected by the company that manufactures and sells the vaccine, as to whether the quality is maintained when the vaccine is stored for a certain period of time, and is set through regulatory procedures. For this reason, even after the expiration date has been set once, if data can be collected on how the quality of the vaccine can be maintained when the vaccine is stored for a longer period of time, based on that data, it will be approved through regulatory procedures. The expiration date may be extended.
(Note) Please be assured that this is not an expired vaccine.
Pfizer vaccine expiry date extension
Due to a notification from the government, the expiration date of the Pfizer vaccine for children has been further extended from 6 months and 9 months to 18 months as of December 2022. Shipping coupons (inoculation tickets)
Initial inoculation (1st and 2nd doses)
Coupons (vaccination tickets) were sent on November 15, 2020 to those who will turn 5 years old in December.
(Note) Coupons (vaccination tickets) for infant vaccinations will be sent to children up to 4 years old. We will no longer send coupons (vaccination tickets) for pediatric vaccinations. (Note) Depending on the postal situation, it may take some time for the package to reach you after it is shipped.
Booster vaccination (3rd time)
It will be shipped around the middle of the month before the month when 5 months have passed since the date of the second vaccination.

Initial inoculation (1st and 2nd doses)
You can make a reservation as soon as you receive the coupon (inoculation ticket).
Booster vaccination (3rd time)
You can make a reservation as soon as you receive the coupon (inoculation ticket).

Reservations can be made on the dedicated reservation site or by telephone.
(Note) Facilities that carry out pediatric vaccinations (aged 5 to 11 years old) are facilities with the notation [Children] at the beginning of the facility name on the reservation site. Please note that we do not carry out at facilities without notation.

Things to bring when inoculating
- Maternal and Child Health Handbook
For pediatric vaccinations (ages 5 to 11), vaccination history is managed in the Maternal and Child Health Handbook. Please bring your Maternal and Child Health Handbook with you on the day of vaccination. In addition, please do not forget to bring the following documents.
- Coupon (vaccination ticket and certificate of vaccination)
- Completed pre-examination form
- Identity verification documents
(Note) A health insurance card is useful as an identity verification document if medical treatment is required.
(Note) On the day of the event, please come in clothes that are easy to expose your shoulders. Inoculate the upper arm.
Considerations
Reissue of coupon (vaccination ticket)
If you lose your coupon (inoculation ticket), etc., you need to apply. (This is the same application form for the 1st and 2nd vaccinations for those aged 12 and over.)
Call center, electronic application, and mail will be accepted. You can also pick up the application form at the 7th floor of Shibuya Ward Office, Community Health Division New Corona Vaccine Counter, Branch Office, and Citizens Service Center.
(Note) If the application is made after the coupon (inoculation ticket) is issued, it will take about 2 weeks from the application. Move-ins may have to wait a few more days.
(Note) Please note that applications are not accepted at branch offices or city resident service centers.

Application for vaccination outside the address
As a general rule, you will be vaccinated at the location on your resident record, but if you have unavoidable circumstances and are vaccinated outside the location on your resident record, you will need to apply. Please submit a copy (in writing) of the vaccination notification and the coupon (inoculation ticket) to the municipality of the place where you wish to be vaccinated.

(Note) If the city determines that maintaining and building the vaccination system is difficult, the application may be suspended by issuing the certificate of completion of vaccination notification outside the address.

If you receive other vaccinations
If you receive other vaccinations before or after childhood vaccination (ages 5 to 11), please leave an interval of 13 days. Other vaccinations cannot be given at the same time.
(Note) Pay attention to the intervals between the MR vaccine for 5-year-olds and the DT vaccine and Japanese encephalitis 2nd-stage vaccination for 11-year-olds. If in doubt, consult your family doctor.
