We are accepting applications for a certificate of vaccination against new coronavirus vaccine infectious diseases, which is intended for use when traveling overseas, such as entering foreign countries.
If you need to prove your vaccinations in Japan, you can use a certificate of vaccination or a vaccination record as proof. If you lose your vaccination certificate or vaccination record, we will issue a domestic vaccination certificate.
*The vaccination certificate for overseas travel can also be used in Japan.
Target audience
Vaccination certificates will be issued to those who have been vaccinated against the new coronavirus using the vaccination ticket issued by Kita Ward, based on the Vaccination Act. Therefore, those who received vaccinations outside Japan (those who received vaccinations not based on the Immunization Act)* are not eligible for issuance.
*From December 9, 2020 (Friday), residents of Kita Ward who fall under the following at the time of application for vaccination certificates can register vaccination records and issue vaccination certificates.
- Those who received the vaccination through a project by the Ministry of Foreign Affairs for temporary returnees of Japanese nationals living abroad
- Those who have participated in clinical trials for new coronavirus infections
- U.S. Forces Japan employees who received vaccinations from the U.S. Forces Japan
All of them are for those who have been vaccinated with vaccines approved in Japan (Pfizer, Moderna, Novavax, AstraZeneca). For details of the conditions, please check the Ministry of Health, Labor and Welfare website (link to an external site) .

If the issuing municipality of the vaccination ticket is other than Kita Ward, Kita Ward cannot issue the vaccination certificate. Please contact the municipality issuing the vaccination ticket.
Example: If you received the first vaccination ticket from Kita Ward and the second vaccination ticket from A Ward, you will need to apply for a certificate of vaccination in both North Ward and A Ward.

There is an electronic delivery and a mail application for a paper certificate (A4 size).
1. Apply with the smartphone app
We will issue a vaccination certificate (electronic version) with a two-dimensional code using a dedicated app on your smartphone.

Items required for electronic delivery
To use electronic delivery, you will need:
- my number card
- Smartphone (iOS13.x or later or Android OS8.x or later)
- 4-digit PIN set when My Number Card is issued
- Passport valid for overseas travel (not required for domestic use)
For those who fall under the following (example), a paper certificate will be issued because the application cannot be automatically issued.

Number of days required for delivery
It takes about 10 days to 2 weeks from the time the application is received until it is returned.
(Note) If the stamps, etc. pertaining to the application or mailing are incomplete, it may take longer. Please apply well in advance of the schedule.

3. Apply at a convenience store (only for those who have a My Number Card)
From July 26, 2020 (Tuesday), it is possible to issue a vaccination certificate using your My Number card at some convenience store terminals.

Conditions for issuance at convenience stores
After July 21, 2022, you have obtained an overseas vaccination certificate from the COVID-19 vaccine certificate app or from a local government in Japan, and your passport number has not changed since that time.
(Note) Those who have never issued a certificate under the above conditions cannot use it.

Q1.Does the vaccination certificate have an expiration date?
A1 .You do not need to apply every time you travel abroad. Currently, there is no expiry date for vaccination certificates.
However, if there are any changes to the information on the certificate (changes to vaccination records, etc., passport expiration date, etc.), both electronic and paper certificates must be reissued.
Q2.I am applying for a passport. Can I apply for an inoculation certificate?
A2 . Please apply after obtaining a passport. Since the passport number is written on the vaccination certificate, it is necessary to present the passport at the time of application (submit a copy if mailed).
About electronic application
Q3.Can I get the child's vaccination certificate on the same smartphone as the parent?
A3 .It is possible for a person with parental authority to obtain a vaccination certificate for their child using their smartphone as a legal representative. In that case, multiple vaccination information will be stored in the same app.
About mail application and vaccination certificate (paper)
Is issuance of Q4. vaccination certificate (paper) only for one application?
A4 .In principle, only one copy will be issued per application. The vaccination certificate (paper) is basically assumed to be presented rather than submitted.
Q5. I lost my vaccination certificate. Could you please reissue it?
