We can make a reservation from vaccination reservation site (the outside site) for Omicron strain .


Please refer to the Omicron Strain Vaccination page for the reservation start date and vaccination start date. An e-mail address is required to make a reservation. However, some people need to register from My Page due to a system malfunction. Please register.
(Note) If a person who received the pediatric vaccine in the first vaccination and is over 12 years old at the time of the third vaccination, the vaccination for those aged 12 and over (Omicron strain vaccine) will be applied.

How to book online

Reservation of conventional strain vaccine (monovalent)

(Note) If you have received the pediatric vaccine for the first time, you will be vaccinated with the pediatric vaccine even if you reach your 12th birthday before the second vaccination. If you are inoculating the first dose at the age of 11 and the second dose at the age of 12, only telephone reservations are accepted for both the first and second doses. Reservations cannot be made through the reservation site.

Those who log in to the dedicated site for the first time

The password you use to log in to the site is your date of birth.
(Example) If you were born on April 15, 1992, the password is "19920415".

(Note) If you want to set your own password, you can do so from My Page after logging in. Also, if you register your e-mail address, you can reset your password yourself if you forget it.
(Note) It has been confirmed that the login fails in rare cases even if the correct vaccination ticket number and date of birth are entered when logging in for the first time. You can log in by pressing the login button again. For details, please refer to Problems when logging in for the first time (PDF487KB) .

Telephone reservation

You can make a reservation by phone at the Shibuya Ward Novel Coronavirus Vaccine Call Center.
When making a reservation, be sure to have your vaccination ticket ready.

(Note) The call center cannot answer medical questions. If you have underlying diseases and are concerned about vaccination, please consult your family doctor.

When special consideration is required

Those who need special consideration, such as those who cannot speak Japanese or who are visually impaired, can consult in advance at the Shibuya Ward Novel Coronavirus Vaccine Call Center (Phone: 0120-045-405 ). Also, at the group vaccination venue, we will support you on the day of vaccination, so please talk to the staff nearby.

Vaccination without reservation

Vaccines can be vaccinated without an appointment at Seibu Shibuya Movida Kan.
(Note) At the vaccination site, priority is given to those who have made a reservation in advance. In addition, if there is a large number of visitors and it interferes with normal vaccination operations, we may cancel vaccinations that do not require a reservation for that day.
(Note) Vaccinations that do not require vaccination tickets are not carried out.

(Note) Vaccinations that do not require an inoculation ticket are not carried out in January. Be sure to bring the coupon (inoculation ticket).

If you do not have a coupon (vaccination ticket)

Please apply for issuance. The application format differs depending on the number of vaccinations. For details, please see the page about the new corona vaccination (1st and 2nd doses) or the page about vaccination for the Omicron strain . It takes about two weeks from application to issuance.

About waiting list vaccination

At the Cultural Center Owada, we are accepting applications for vaccination with the Pfizer bivalent vaccine for the Omicron strain in the reservation frame that has been canceled.
