Purpose of inoculation and supply of vaccine

Even if infected by inoculation, it is expected to prevent aggravation. In addition, it is expected that more people will be vaccinated, reducing the number of severe cases and deaths, and reducing the burden on medical institutions.
The vaccine secured by the country will be distributed to Shibuya Ward through the city. The national government assumes that the distribution cycle is two to three times a month. After that, the city prepares the vaccine for inoculation at medical institutions.

Target audience

  1. Those who have a resident card in the ward (persons over 12 years old for currently approved vaccines)

Application for issuance of coupon (inoculation ticket)

Coupons (vaccine tickets) for the first vaccination (1st and 2nd doses) will not be sent automatically. If you have not been vaccinated and do not have a coupon (inoculation ticket), please be sure to apply for issuance.

Application method

Call center, electronic application, and mail will be accepted. You can also pick up the application form at the 7th floor of Shibuya Ward Office, Community Health Division New Corona Vaccine Counter, Branch Office, and Citizens Service Center.

(Note) The call center only accepts applications by the applicant himself/herself, and cannot accept applications by proxy.
(Note) If the application is made after the coupon (inoculation ticket) is issued, it will take about 2 weeks from the application. Move-ins may have to wait a few more days.
(Note) Even those who are not listed on their resident card, or who have been victims of domestic violence or stalking, may be required to wait a few days for a new number to be issued.
(Note) Please note that applications are not accepted at branch offices or city resident service centers.

Application for vaccination outside the address

As a general rule, you will be vaccinated at the location on your resident record, but if you have unavoidable circumstances and are vaccinated outside the location on your resident record, you will need to apply. Please submit a copy (in writing) of the vaccination notification and the coupon (inoculation ticket) to the municipality of the place where you wish to be vaccinated.

(Note) If the city determines that maintaining and building the vaccination system is difficult, the application may be suspended by issuing the certificate of completion of vaccination notification outside the address.
(Note) You do not need to apply for out-of-address vaccination if you receive the new coronavirus vaccine at work.
(Note) Please note that applications are not accepted at branch offices or resident centers.

Application at window Please submit a copy (in writing) of the address out-of-town vaccination notification and the coupon (vaccination ticket) to the counter in charge of the new coronavirus vaccine on the 7th floor of the Shibuya Ward Office.

(Note) Please note that the call center can only accept inquiries, but cannot accept applications.
(Note) The destination of the mail application will be the address of the business operator entrusting the work.
(Note) Please prepare a self-addressed envelope with an 84-yen stamp affixed to a standard-size inner envelope that can fit A4 paper folded in three. Also, please include the address and name of the recipient.

When application is required

If you meet the following unavoidable circumstances, you will need to apply for vaccination outside your address.

  • Those who have evacuated from outside the city due to domestic violence, stalking, etc. and have not moved their resident card to Shibuya City
  • Pregnant women returning home for childbirth
  • unaccompanied worker
  • Students lodging from remote areas
  • People who live in Shibuya Ward but have not transferred their resident card

If your application cannot be accepted

Please note that applications will not be accepted if any of the following apply.

  • If you do not live in Shibuya City, but are working or studying in Shibuya City
  • When it is difficult to inoculate at the resident card location, etc.

If no application is required

If the following apply, you do not need to apply for vaccination outside your address.

  • Inpatients/Residents
  • Persons with underlying diseases to be vaccinated under the supervision of the attending physician

About alternating inoculations

The second dose of vaccine can be the same type of vaccine used for the first dose or a different type.

(Note) If you have returned from overseas and have been vaccinated with vaccines other than those approved by the Japanese Ministry of Health, Labor and Welfare (Pfizer, Moderna, AstraZeneca), be sure to consult your family doctor.
(Note) If you are to receive alternating vaccinations, please make a reservation so that the first vaccination date and the interval are at least 27 days.

Concurrent vaccination with other vaccines

The new corona vaccine and other vaccines can't be vaccinated at the same time. Be sure to inoculate at least 13 days after inoculating one. However, simultaneous vaccination is possible only for influenza vaccine. About extension of the expiration date of the new corona vaccine

The expiration date of a vaccine is set based on data collected by the company that manufactures and sells the vaccine, as to whether the quality is maintained when the vaccine is stored for a certain period of time, and is set through regulatory procedures. For this reason, even after the expiration date has been set once, if data can be collected on how the quality of the vaccine can be maintained when the vaccine is stored for a longer period of time, based on that data, it will be approved through regulatory procedures. The expiration date may be extended. (Note) Please be assured that this is not an expired vaccine.

Pfizer vaccine expiry date extension

As of April 2022, the expiration date of the Pfizer vaccine has been extended from 9 months to 12 months due to a notification from the government, and on August 19, 2022, the vaccine for 12 years old and older will be 12 months old. It was further extended from 1 month to 15 months. Therefore, when the vaccine with the following lot number is vaccinated, a vaccine sticker with an expiration date of 9 months will be attached to the new corona vaccine vaccination certificate, but the correct validity after the change will be applied. Deadlines are:

1. Sending coupons (vaccination tickets)

The city has sent an envelope containing a new corona vaccine coupon (vaccination ticket) to all eligible people for vaccination. If you have not been vaccinated and do not have a coupon (inoculation ticket), please apply for issuance of a coupon (inoculation ticket).

2. Confirmation of vaccination site

It will be a "group vaccination venue" at a facility in the city. For details, please refer to the page of the list of new corona vaccination facilities .

3. Vaccination appointment

Pre-booking is required for vaccinations. Reservation methods are online reservation and telephone reservation. For more information, please visit the new corona vaccination reservation page.
(Note) Please check whether the vaccination reservation has been successfully made on the reservation site. You can check the reservation status from "My Page" on the new corona vaccine reservation site (external site) . If there is no reservation date and time on "My Page", the reservation may not have been made properly.

4. Filling in the preliminary examination form

A vaccination form must be completed prior to vaccination. Please consult your family doctor before completing the form, if necessary.

Please do not forget to bring the following documents on the day of vaccination.

Precautions regarding reservations and inoculations

About the second vaccination (in the case of Pfizer vaccine)

After booking the first vaccination, you can book the second vaccination in succession. Please make a reservation so that the interval between the first scheduled vaccination date is 3 to 6 weeks. Please refer to the precautions regarding the second reservation (PDF 383KB) .
(Note) In the case of Moderna vaccine, the vaccination interval is 4 to 6 weeks.

(Note) If there is a vacant slot, you can make a reservation even for a schedule before the first vaccination date. In that case, the first dose will be given on the earlier day, and the second dose will be given on the later day.
(Example) If the first reservation date is August 20 (vaccination date), the next available reservation date is July 9 to July 30 or September 10 to October 1.

About inoculations under the age of 16

If you are under the age of 16 and are vaccinated, you must be accompanied by a guardian in principle. However, only for those who are junior high school students or older, if the parent's consent to the vaccination can be confirmed in the parent's signature column (PDF 140KB) on the preliminary examination form at the time of reception on the day of vaccination, it is not necessary to accompany the person. is not.

Other matters related to inoculation

If you wish to receive the vaccination at home

In spite of wanting to be vaccinated for reasons such as home care and medical treatment in the city, it is difficult to vaccinate at the group vaccination venue or individual medical institution, and the vaccination is received by a family doctor's house call. We will guide you through home-visit vaccinations for those who cannot. For applications, please contact the Regional Health Division Vaccination Section (Phone: 03-3463-1269).

Obtaining consent for vaccination

Vaccines will only be administered with the consent of the vaccinated person.
(Note) If you are currently undergoing treatment for some kind of illness or if you have any concerns about getting vaccinated due to your physical condition, please consult with your family doctor before considering vaccination.

When special consideration is required for inoculation

Those who need special consideration, such as those who cannot speak Japanese or who are visually impaired, can consult in advance at the Shibuya Ward Novel Coronavirus Vaccine Call Center (Phone: 0120-045-405 ). Also, at the group vaccination venue, we will support you on the day of vaccination, so please talk to the staff nearby.

Precautions after inoculation

Vaccination is expected to prevent the development of COVID-19, but the extent to which it protects others from infecting others is not yet known. Also, at the stage where vaccination is gradually progressing, many people will not be able to receive the vaccine immediately, and both those who have been vaccinated and those who have not will have to live a social life together. .
Even after vaccination, please continue to cooperate in preventive measures against infectious diseases, such as avoiding crowds, close contact, and airtightness, wearing a mask, washing hands with soap and disinfecting with alcohol for hand disinfection.
