- Must be 12 years old or older and live in Meguro Ward
- Being unable to go to the vaccination venue because you are constantly bedridden or in a similar condition
- Have permission to vaccinate from your family doctor
- It is possible for family members to be present at the time of home-visit vaccination.

Notes
- Applications are not first-come, first-served
- Please have your vaccination card ready when you apply.
- After receiving the application, we will coordinate the schedule and contact you when the date and time of the visit are decided.
- Depending on the status of your application, it may take some time to guide you through the vaccination process.
- Currently, the national vaccination period is until March 31, 2023. Those who wish to be vaccinated against the Omicron strain and have not received the first (1st and 2nd) vaccination must complete the first vaccination by the end of December 2022 at the latest.
