Target audience
Those who live in Nerima Ward, wish to be vaccinated, and meet all of the following criteria.
- Those who are constantly bedridden during home care, or those who are in an equivalent condition (equivalent to Level 4 or 5 nursing care, Grade 1 or 2 physical disability certificate, Love certificate 1 or 2)
- Those who have difficulty accompanying family members or supporters to the vaccination venue or moving support
- Those who find it difficult to get to the vaccination site even if they use a welfare taxi with a lift provided by the ward
- Those who cannot be vaccinated by a family doctor's home visit, and who have received permission from their family doctor to be vaccinated
- Those who can be accompanied by family members or supporters and follow-up after vaccination

(1) Those aged 12 and over
● 1st and 2nd inoculations: conventional vaccine (planned to be manufactured by Pfizer)
● 3rd and subsequent vaccinations: Vaccine for Omicron strain (planned to be manufactured by Pfizer)

(2) 5 to 11 years old

Application method
A family member or supporter, etc., must fill in the necessary items on the "Application for Home-visit Vaccination (New Coronavirus Infectious Disease)" and submit it to the Residents' Vaccination Section by mail or email .
About the schedule and time to visit, the contractor will contact you by phone about 5 to 7 days before the vaccination date.

● The second vaccination will be given on the same day of the week three weeks later. No new application is required for the second vaccination.
