It is located at the bottom right of the new coronavirus vaccination ticket in Meguro Ward.

Please apply if you meet all of the following.

• Those who received the new corona vaccine
• Those who wish to have their vaccination certificate or vaccination record reissued due to loss, etc.
• Those who are registered as residents of Meguro Ward on the date of vaccination

Note: If the vaccination record cannot be verified by the National Vaccination Records System (VRS), it cannot be issued.
In addition to priority vaccinations for medical workers, occupational vaccinations, large-scale national / metropolitan vaccinations, and those who received vaccinations outside of Meguro Ward's vaccination venues, it takes 3 months for vaccination records to reach Meguro Ward from each vaccination venue. It may take a while, so you may have to wait for a long time until it is issued.
