If you fall under any of the following categories, you need to apply for the issuance of a vaccination ticket.

• Moved to Meguro Ward (note)
• lost vaccination ticket
• It is past the vaccination period, but the vaccination ticket has not arrived.
• I tried to inoculate, but I could not inoculate and used it only for a preliminary examination
• The latest new corona vaccine was overseas
• The most recent COVID-19 vaccination was a COVID-19 vaccination project for Japanese nationals living overseas (vaccination at Haneda Airport and Narita Airport).

(Note) Vaccination tickets issued by the local government before moving in cannot be used. After moving in, please apply for issuance of a vaccination ticket from Meguro Ward. Please note that even if you received vaccinations in the municipality before moving in, the next vaccination ticket will not be sent automatically.

If you have been vaccinated one or more times, please prepare and apply for the vaccination record (certificate of vaccination, vaccination record book, vaccination certificate, etc.) at hand.
If you apply online, the vaccination ticket will be sent to the address listed on your resident card.
If you wish to change the delivery address, please apply by mail or at the counter and follow the procedure for changing the delivery address.

Documents required for application are as follows.

• A copy of the vaccination record (certificate of vaccination, vaccination record, vaccination certificate, etc.) (only for those who have been vaccinated one or more times)
• New Corona Vaccination Ticket Issuance Application Form

Please bring the following identity verification documents when applying at the counter.

• Personal identification documents of the person to be vaccinated (driver's license, health insurance card, etc.)

(Note) In the case of proxy application at the window, identification documents of the proxy are required along with the identity verification documents of the recipient.

As a general rule, vaccination tickets will be sent to the address listed on your resident card. For unavoidable reasons, if you would like to have your documents sent to an address other than the one listed on your resident card, please also send us the necessary documents to change the address.

We will ship them sequentially as the vaccination time approaches.
For those who have already reached the vaccination period, we will send it out in about a week from the application reception.

When you receive the vaccination ticket, please make a reservation for vaccination on LINE or by phone.

If for some unavoidable reason it is difficult to get vaccinated at the location on your resident record, you can receive the vaccination in Meguro Ward by notifying Meguro Ward in advance.
If you use the Meguro-ku vaccination ticket to receive vaccinations in other municipalities, please apply for out-of-address vaccinations to the municipality where you will be vaccinated.

• Pregnant women returning home for childbirth
• unaccompanied worker
• Students lodging in remote areas
• Victims of child abuse and similar acts such as DV and stalking
• Any other person recognized by the municipality as having unavoidable circumstances

Please prepare and apply for the vaccination ticket for the desired vaccination schedule issued by the local government where you have your resident card.
If you have been vaccinated one or more times and the most recent vaccination record is not listed on the vaccination ticket, please also prepare a document that shows the vaccination record (vaccination certificate, vaccination record book, vaccination certificate, etc.).
(Example) If you wish to be vaccinated for the 3rd time, you will need the 3rd vaccination ticket and the 2nd vaccination record.

Documents required for application are as follows.

• A copy of the desired vaccination ticket issued by the municipality with the resident card
• A copy of the vaccination record (certificate of vaccination, vaccination record book, vaccination certificate, etc.)
• Out-of-address notification of vaccination (New Corona vaccination)
