It takes about two weeks from the time the documents are received to the time they are issued. Please note that it may take longer depending on the application status.

2 If your passport has a maiden name, another name, or another name written on it

Please submit a copy of an identification document that confirms your maiden name, alternate name, and alias.

FAQ

Q1. I already have a "Certificate of Vaccination" and "Record of Vaccinations", but do I need to re-issue a new "Certificate of Vaccination" for proof in Japan?

As a general rule, you don't need to start over. However, there is a possibility that a "certificate of vaccination" may be required by the restaurant or event organizer. After confirming with the certification destination, please apply if necessary.

Q2.Is it possible to apply for a certificate of vaccination only for "overseas use"?

There are only two types of national forms, "domestic" or "overseas and domestic", so please select one of them.

In addition, when traveling overseas, it can be used without problems in "overseas and domestic use".

Q3.If I apply for a paper version , how will my name be written?

In the case of "domestic use", it is written based on the name information listed in the basic resident register. If you wish to use your maiden name, alternate gender, or alias, please attach a copy of your identity verification document (*) that confirms your maiden name, alternate name, or alias.

In the case of "overseas and domestic use", the name information is written based on the MRZ (machine readable part) at the bottom of the attached passport copy. Even if you have a middle name, it will not appear on your vaccination certificate if it is not listed on your MRZ.

Q4.How long does it take for the electronic version (application) and paper version (mail) to be issued after application?

In the case of the electronic version (app), it can be issued in about 5 minutes.

In the case of a paper version (by mail), it takes about two weeks from the time the document arrives to the time it is issued. Please note that it may take longer depending on the application status.

Q5.Is it possible to apply for vaccination certificates for family members (spouse, children, etc.) in the same household together with the certificate for the individual in one envelope?

If you need to apply for multiple people, you can mail them together in one envelope. In addition, you can also return the reply envelope in one envelope in the same way.

However, regarding the stamps affixed to the envelope for submitting the application form and the self-addressed envelope, please affix a stamp that matches the shape of the envelope and the weight of the document .

Q6.If I want to obtain an "overseas version" of a vaccination certificate at a convenience store, why do I have to obtain an "overseas version" of the vaccination certificate from the Adachi Ward Office or the "Vaccination Certificate App" in advance?

The "overseas version" of the vaccination certificate has a column to enter your passport information. This is because it is necessary to register by either the department in charge or the "Vaccination Certificate App".
