Please use documents such as test result reports issued by medical institutions and test result notification emails from the positive person registration center.
For those who are subject to notification of outbreak
Various notifications will be sent to those who have been diagnosed with the new coronavirus infection and are subject to notification of outbreak based on the law on the prevention of infectious diseases and medical care for patients with infectious diseases.
(Note) Please note that it will take about a month (depending on the number of positive cases, etc.) to receive the notification.

It can be used for medical insurance claims to insurance companies, so please make use of it.

About the notice of the end date of home medical treatment
If the My HER-SYS medical treatment certificate cannot be used, we will issue a home medical treatment end notice to those who request it.
*If the purpose is to make a medical insurance claim, please use MyHER-SYS as a proof of medical treatment as much as possible.
The Life Insurance Association of Japan and the General Insurance Association of Japan do not require proof of the end date of medical treatment if the period of medical treatment is within the period that conforms to the criteria for canceling medical treatment stipulated by the Ministry of Health, Labor and Welfare.
〇 Eligible people
Those who fall under the following
1. Those who have been diagnosed with a new type of coronavirus infection (Those who are not subject to the notification of outbreak are not applicable.)
2. Those who received health observation from Bunkyo Ward during medical treatment
3. Those who received medical treatment at home (Those who transitioned to accommodation treatment or hospitalization during treatment are not applicable.)
4.Those whose medical treatment period exceeds the medical treatment cancellation criteria stipulated by the Ministry of Health, Labor and Welfare
5. Those who have been diagnosed with a suspected disease (deemed positive)

〇 Notes
- It takes about one month from the time the application is received until the notification is issued.
- We will only issue notifications for the period you were in home care. We are unable to issue a certificate for the period of stay at an accommodation facility provided by the Tokyo Metropolitan Government or the period of hospitalization at a medical institution.
- Certificates in the form of individual insurance companies cannot be issued.
For those who received accommodation treatment
For those who have completed the accommodation treatment, we will issue a certificate of accommodation treatment to those who wish.

For those who have been hospitalized or discharged
About hospitalization recommendation letter, hospitalization extension recommendation letter, hospitalization extension notice
Based on the Act on Prevention of Infectious Diseases and Medical Care for Patients with Infectious Diseases, we are sending this to those who have been hospitalized due to the new coronavirus infection.
(Note) If the period described in the notification is insufficient for the actual hospitalization period, a notification of extension will be sent together with the notification of the determination of public expenses.
About medical expenses public expenditure decision notice (or medical expenses payment decision notice)
Based on the "Act on Prevention of Infectious Diseases and Medical Care for Patients with Infectious Diseases", hospitalization costs due to new coronavirus infections, excluding insurance coverage, will be borne by public funds.
An application is required for the public expense burden, so please submit the consent form (PDF file; 71KB) that will be sent together with the hospitalization recommendation form as soon as possible after you are discharged from the hospital.
We will send you a notice after the public expense burden is decided.
(Note 1) The start of public expenditure is the date of hospitalization by recommendation, and the end is the date when it is confirmed that no pathogens are present. It does not necessarily match the date of admission and discharge.
(Note 2) Fees not covered by health insurance, such as extra bed charges, are not covered by public funds.
(Note 3) If you do not apply (if you do not submit a consent form), you will be required to pay the full amount. Please pay based on the bill of the medical institution.
