About payment of sickness allowance for National Health Insurance subscribers due to new coronavirus infection

If you are an insured person of the Ota City National Health Insurance and have been infected with the new coronavirus infection or have symptoms such as fever and are suspected to be infected, you will need to stay at a company, etc. for at least 4 days, including 3 consecutive days, for medical treatment. If you were unable to receive payment of all or part of your salary, etc. for the period of absence from work, you will receive an injury and sickness allowance. The application period has been extended.

Payment requirements

・ Target person ・
Those who correspond to all of the following
1 Ota Ward National Health Insurance Insured Persons
2 Those who receive salary, etc. from their place of employment
3 If you are suspected of being infected with the new coronavirus infection or have symptoms such as fever,
Those who have been absent from the company, etc. for 4 days or more , including 3 consecutive days, due to medical treatment
4 Those who are not paid all or part of their salary for the period they were unable to work
(Note 1) If you are enrolled in social insurance instead of Ota City National Health Insurance,
Please contact your social insurance company.
(Note 2) Sole proprietors are not eligible.
(Note 3) The aftereffects of the new coronavirus infection are not eligible for payment.

・Payment period・
A period of inability to engage in labor from the day on which 3 consecutive days have elapsed counting from the date on which the employee is no longer able to engage in labor.

・Payment amount・
(Total amount of salary income for the most recent three consecutive months ÷ number of working days) x 2/3 x number of days (number of days subject to payment)
(Note 4) If you can receive all or part of your salary, etc., the amount of payment may be adjusted or may not be paid.
(Note 5) There is an upper limit to the amount of payment per day.

・Applicable period・
The period from January 1, 2020 to March 31, 2023 when you cannot work due to medical treatment (however, if hospitalization continues, up to 1 year and 6 months)
・The application period has been extended from December 31, 2022 to March 31, 2023.

・Prescription・
It will be two years from the day after the day you can no longer engage in labor.
(If December 1, 2020 is the date when you can no longer work, you can apply for the Injury and Sickness Allowance until December 1, 2022.)

・How to apply・
Application must be made by mail .
There is a designated form for the application form, so please be sure to contact the National Health Insurance Benefits Section in advance before applying.

1 Injury and Sickness Allowance Payment Application Form (for entry by householder)

2 Injury and Sickness Allowance Payment Application Form (for insured persons)
(Note 6) A certificate from the business owner is required in the business owner entry column of the application form.

3 Injury and Sickness Allowance Payment Application Form (for business owners to fill out)

(Note 7) It is no longer necessary to submit the 4 Injury and Sickness Allowance Payment Application Form (to be filled out by a medical institution), but instead, the employer must fill in the 2 Injury and Sickness Allowance Payment Application Form (to be filled in by the insured person) proof of is required.

5 Pledge and Consent

After completing the above three application forms and the pledge/agreement, please enclose a copy of 6.7.8.9 and mail it.
Please use a seal that is stamped with red ink.

6A copy of an identification document with a photo issued by a public institution (driver's license, passport, etc.)

7Copy of health insurance card

8 Something that can confirm the transfer account (copy of passbook, copy of cash card, etc.)

9 Documents that show the salary for the relevant period (salary statement, etc.)
