About the medical treatment certificate associated with the nationwide uniform notification of new coronavirus infections from September 26, 2022
From September 26, 2022, due to the nationwide limited coverage of the notification of the outbreak of the new coronavirus infectious disease, medical treatment certificate (medical treatment information statement issued by Itabashi Ward Public Health Center and medical treatment certificate at My HER-SYS ) has been limited.
For those who have been diagnosed with a new coronavirus infection at a medical institution after September 26, 2022, those who fall under any of the following 4 types are subject to notification of outbreak, in principle. Medical treatment certificates cannot be issued to those who are not subject to notification of outbreaks (those who do not fall under any of the following four types).
As a general rule, those who are not subject to notification of the outbreak will be notified of the positive person registration center by the medical institution at the time of diagnosis. There is no contact from the public health center to those who are not subject to the outbreak notification. If you are unsure whether you are subject to an outbreak notification, please contact the medical institution where you received the diagnosis.
In addition, in principle, those who have been diagnosed with a new coronavirus infection at a medical institution before September 25, 2022 will be subject to an outbreak notification.
If you need a certificate of medical treatment for reporting an outbreak, please refer to the following section on this page, "Certificate of medical treatment issued (displayed) from My HER-SYS (applicable only for reporting an outbreak)".
[Four types of new coronavirus infectious disease outbreak notification target]
(1) Those aged 65 and over
(2) Those requiring hospitalization
(3) Those who are at risk of aggravation and need to be administered new coronavirus infectious disease therapeutic drugs,
or
Those who are at risk of aggravation and who newly need oxygen administration due to the new coronavirus infection
(4) Those who are pregnant
About the medical treatment certificate issued (displayed) from My HER-SYS (Applicable only for outbreak notification)
From April 27, 2020, My HER-SYS added a function that allows you to display and prove the date of diagnosis by your own operation, so please use it.
This certificate is valid if the period of accommodation treatment or home treatment is within the period according to the medical treatment cancellation criteria of the Ministry of Health, Labor and Welfare. The Ministry of Health, Labor and Welfare indicates that it can be used to claim insurance claims from insurance companies affiliated with the Life Insurance Association of Japan or the General Insurance Association of Japan.
Currently, due to the rapid increase in the number of positive people, it may take about a month from the time you apply for the medical care information statement issued by the public health center to the time it is sent. We recommend using a certificate.
Note: 1 Please check with the place where you submitted the certificate of medical treatment to see if it is possible to carry out procedures with the certificate of medical treatment issued by My HER-SYS.
Note: 2 Those who can display a certificate of medical treatment on My HER-SYS are those who have undergone tests (PCR test, antigen test, etc.) and have been diagnosed with a new coronavirus infection by a doctor.
If you are a "special suspected patient (deemed positive)", you cannot display the medical treatment certificate on My HER-SYS, so please refer to the "Medical Information Statement (Application Required)" below and fill in the medical information. Please apply for issuance of the certificate.
Note: 3 My HER-SYS is a health management system of the Ministry of Health, Labor and Welfare that allows people who test positive to enter their own health status and that of their family members using a smartphone or computer. If you do not have the URL and HER-SYS ID required to register for My HER-SYS and wish to use My HER-SYS, please contact the Itabashi Public Health Center (03-3579-2321).

How to view the medical certificate on My HER-SYS
You can display the medical certificate from My HER-SYS by the following operation.
For information on how to submit a certificate of medical treatment for insurance claims, etc., please check with the place where the certificate of medical treatment is submitted.
Note: 1 There is no print function on the medical treatment certificate screen of My HER-SYS. If you want to print it, use the screenshot (screen save) function of your smartphone, save it once, and then print it.

Please note that we will not be able to issue medical care information statements to those who are not subject to notification of the outbreak of the new coronavirus infection, even if they have applied.
Items listed in the Medical Care Information Statement for Novel Coronavirus Infectious Disease Patients
- Date of first medical examination (day of first visit to a medical institution for novel coronavirus infection)
- Diagnosis date (date of diagnosis of new coronavirus infection)
- Date of onset (date when symptoms of novel coronavirus infection appeared. Sample collection date for asymptomatic patients)

Note: 1 Regarding the items listed in the medical care information for patients with the new coronavirus infectious disease, information that cannot be grasped by the public health center will not be listed due to the simplification of the notification of the outbreak by the Ministry of Health, Labor and Welfare. Please note.
Note: 2 The period of recuperation is the period during which the person is considered to be infectious with the new coronavirus infection, and does not necessarily match the period during which the person had symptoms.
Note: 3 In principle, the end date of medical treatment will be within the period (see * below) in accordance with the criteria for canceling medical treatment set by the Ministry of Health, Labor and Welfare. Medical treatment due to symptoms of diseases other than new coronavirus infections, voluntary medical treatment, and isolation periods are not covered.

The medical treatment period indicated by the Ministry of Health, Labor and Welfare is as follows. In addition, due to the notification of the Ministry of Health, Labor and Welfare, the criteria for the period of medical treatment have changed from September 7, 2020. See below.
In principle, the end date of medical treatment to be entered at the time of application will be a date within the following medical treatment period.

Until 7 days have passed since the day of onset and 24 hours have passed since the symptoms resolved.

For those with symptoms, 10 days from the date of onset and 72 hours after symptoms have subsided. For asymptomatic patients, in principle, 7 days from the date of sample collection.If symptoms occur during treatment, the treatment period for "symptomatic patients" will start from that day.
If the period of medical treatment exceeds the period of medical treatment indicated by the Ministry of Health, Labor and Welfare, when applying electronically, in the item of whether or not to extend medical treatment, "Symptoms will be relieved during the period according to the medical treatment cancellation criteria of the Ministry of Health, Labor and Welfare." Please select “I extended my medical treatment without delay” and enter the reason for the medical treatment extension. In that case, please note that the Itabashi Public Health Center may contact you for confirmation. - The information you enter when you apply electronically may not necessarily be included in the Medical Information Statement. The Medical Care Information Statement will be issued after comprehensively judging the content of the application, the Ministry of Health, Labor and Welfare's criteria for canceling medical care, and other circumstances.
