Confirmation of documents and notification of results
If there are any incomplete documents or omissions in the application form, we will contact you by phone or return the application form, so it will take longer than usual to receive notification of the screening results.
Before sending the application form by post, please check once again whether there are any defects in the required documents (Be sure to check if there are any missing documents before applying by referring to the checklist on the front side of the reduction exemption application form. Target household
- Households where the main livelihood supporter (Note 1) died or suffered serious injury or illness (Note 2) due to the novel coronavirus infection
- Households whose income in 2022 of the main livelihood supporter (Note 1) has decreased due to the impact of the new coronavirus infection and meets all requirements from (1) to (3) of the requirements
(Note 1) In principle, the main livelihood supporter is the head of the household. However, if it is judged comprehensively from the income and income situation and it is recognized that the household mainly maintains the livelihood, it is a person other than the household head.
(Note 2) Serious injury or illness refers to cases where the symptoms of the new coronavirus infection are extremely severe, such as being recognized as having been treated for one month or more.

(1) The amount of decrease in any of the business income (business income, real estate income, forest income or salary income) in 2022 (Reiwa 4) is the business income in 2021 (Reiwa 3) 30% or more (business income, etc. does not include various benefits such as monthly support money and cooperation money from the country and prefectures).
(2) The total income amount in 2021 is 10 million yen or less.
(3) The total amount of income in 2021 other than income related to decreased business income is 4 million yen or less.
However, even if the requirements are met, the income in 2021 pertaining to the expected decrease in (1) is less than 1 yen, or the main livelihood supporter in 2021 If the total income amount is less than 1 yen , the amount of exemption cannot be calculated in the calculation of exemption amount (PDF: 100KB) , so it is not eligible for exemption.

Application method
Applications will be accepted by mail to prevent the spread of infection.
Even if you visit the agency, you cannot review approval or disapproval at the counter. In addition, there is a possibility that you may have to wait for a long time because the window is crowded with applications for joining / withdrawal. Application form and documents required for application
Households who have died or suffered serious injuries or illnesses
- Exemption application form
- A copy of the death certificate or doctor's certificate of the main breadwinner
- A copy of the identity verification document of the householder (or applicant) (license, passport, etc.)
(Note) If you are applying for a reduction or exemption due to the death or serious injury or illness of your main livelihood supporter, please contact the person in charge below before preparing the application documents.

- A copy of something that shows the amount of income in 2022 (withholding slip, final tax return, sales ledger (annual), etc.)
- A copy of the tax return for 2021 for all National Health Insurance subscribers (if only employment income, a withholding slip for 2021 is also acceptable)
- A copy of the identity verification documents of the head of the household (driver's license, passport, etc.)
- If you have discontinued your business or have been dismissed from your employer, a copy of a document certifying that you have discontinued your business (notice of business closure, transfer notification, etc.) or a copy of a document certifying your retirement (retirement certificate, separation from employment, etc.)

Notification of application results
After you apply, we will send you a notification of the result of the reduction exemption examination in about 1 to 2 months. After that, if reduction or exemption is approved, we will send you a notice of decision to change insurance premiums, and formally decide on reduction or exemption of insurance premiums. Depending on the number of applications, it may take longer than this.
Insurance premium for 2021
Of the insurance premiums for 2021, the premiums for which the payment deadline is set between April 1, 2022 and March 31, 2023 may be eligible for reduction or exemption.
