Please check with the medical institution where you diagnosed whether or not you have submitted an outbreak report.
Even if you are registered with the positive person registration center, it cannot be issued to those who are not eligible to submit an outbreak report.
Please use the medical treatment certificate of "My HER-SYS"
If you need a certificate of medical treatment for insurance company medical insurance claims, etc., you can obtain it from MyHER-SYS. For details, please see the page of the Ministry of Health, Labor and Welfare below.

* MyHER-SYS ID will be notified by SMS when the medical institution diagnoses it. If you do not know your ID, please contact us at the number below (03-3880-5747).
*MyHER-SYS medical certificate has already been discussed by the Ministry of Health, Labor and Welfare with the Japan Medical Association, the Financial Services Agency, the Life Insurance Association, and the General Insurance Association of Japan.
Those who cannot use My HER-SYS
Those who have been diagnosed at a medical institution (patients who have been notified of the outbreak) who cannot use My HER-SYS (those who do not have a terminal such as a smartphone or computer that can use My HER-SYS) , Please apply for the medical treatment information notification issued by the ward from the link below. Please note that it will take about a month to issue.
It cannot be issued to those who are not eligible to submit an incident report. Please check with the medical institution where you diagnosed whether or not you have submitted an outbreak report.
