In addition, from September 26, 2022, the target of the patient outbreak notification will be (a) those aged 65 and over, (b) those who require hospitalization, and (c) those who are at risk of aggravation and who are infected with the new coronavirus infection. It was decided to request the submission of an outbreak report limited to four types : (d) pregnant women who are judged by doctors to need treatment drugs or new oxygen administration .
Therefore, after September 26, 2022, medical certificates will not be issued to patients with new coronavirus infections other than the above four types (a) to (d). .
In addition, it is possible to issue a medical treatment certificate for those who have been treated for a new coronavirus infection before September 25, 2022 and submit an outbreak notification.

When using for insurance claims, in response to a request from the Financial Services Agency on September 1, 2022, the Life Insurance Association will issue a medical certificate to each member company when paying benefits, etc. It is well known to consider office work construction that does not require public health centers. Therefore, if it is possible to claim insurance by a method other than a medical certificate, please use that method.
In addition, even if it is necessary to present a certificate of medical treatment to the company, etc., please respond by issuing a certificate of medical treatment by HER-SYS below, and do not request the submission of the certificate of medical treatment (paper) unless necessary. What is a home care certificate?

It is a document that proves that you were diagnosed with a new coronavirus infection and were receiving medical treatment. For issuance, there are two methods: 1. HER-SYS issuance of a medical treatment certificate, and 2. application for a home medical treatment certificate issued by Nakano Ward.

*According to a notification from the government, the Life Insurance Association and the General Insurance Association of Japan will end the accommodation or home medical treatment if the period of accommodation or home medical treatment is within the period according to the medical treatment cancellation standards of the Ministry of Health, Labor and Welfare. We have decided not to require proof of date ( notification of documents certifying accommodation or home medical treatment ). Therefore , only the start date of medical treatment is indicated on the certificate of medical treatment .

1. How to issue a medical treatment certificate by HER-SYS

*Initial registration is required to log in. Please refer to How to Use (opens in a new window) .
*Currently, there is no printing function for the medical treatment certificate of MY HER-SYS. If you want to print it, please use the screenshot (screen save) function of your smartphone, save it once, and then print it.

2. How to apply for a home care certificate issued by Nakano Ward

Please use this application method only when it is truly necessary, as described in the points to note at the time of issuance .

*If Ward 1 issues a home care certificate, it will take one month from application to issuance of the certificate. Please use the issuance of a medical certificate by "HER-SYS" unless it is truly necessary.
*2According to a notification from the government, the Life Insurance Association of Japan and the General Insurance Association of Japan have decided to cancel accommodation or home medical treatment if the period of accommodation or home medical treatment is within the period according to the criteria for canceling medical treatment set by the Ministry of Health, Labor and Welfare. We have decided to handle it so that proof of the end date is not required ( notice about documents certifying accommodation or home treatment ). Therefore , only the start date of medical treatment is indicated on the certificate of medical treatment .
*3 A certificate stating the end date of medical treatment can be issued only when the standard medical treatment period has been exceeded.
*4 Each person can only apply once to issue a certificate of home medical care , so please keep the notification you receive at home in a safe place. If you need to submit more than one, please submit a "copy". It may take up to one month to issue a reissue request.
*5 From September 26, 2020, new patients with new coronavirus infections will be subject to outbreak notifications, and from September 26, 2020, patients will be subject to outbreak notifications (a) 65 years old and over (b) those who require hospitalization, (c) those who are at risk of aggravation and who are judged by a doctor to be in need of new coronavirus infection treatment drugs or new oxygen administration, (d) pregnant women , The medical treatment certificate is also issued only to eligible persons. What is the standard treatment period?

The standard treatment period is 7 days from the day of onset as day 0, and 24 hours after the day when the symptoms were relieved (7 days from the day of examination if no symptoms). The date of onset is determined by a doctor upon examination at a medical institution. If you are unsure of the date of onset, please contact your healthcare provider. *If your medical treatment is taking longer than this, you will need to provide a reason for the extension . Please note that self-treatment after discharge from the hospital or hotel will not be accepted as an extension.

Those who can apply

Those who meet all the conditions from 1 to 5 are eligible.

4.Those who have completed the medical treatment period *We cannot prove the period of close contact 5.(a) Persons aged 65 or over, (b) Persons requiring hospitalization, (c) Those at risk of aggravation and new type Those who fall under any of the following four categories: (d) Pregnant women, or (d) Pregnant women who are deemed by a doctor to be in need of new oxygen administration or administration of drugs to treat coronavirus infections.
