About the new coronavirus infected person information grasping and management support system (HER-SYS)

Information on outpatient medical institution user registration for the system for identifying and managing information on people infected with the new coronavirus (HER-SYS)

The new coronavirus infectious disease information grasping and management support system (hereinafter referred to as HER-SYS) collects information on patients, etc. related to the new coronavirus infectious disease and shares it with the relevant parties to prevent the spread of the new coronavirus. A system developed by the Ministry of Health, Labor and Welfare to effectively implement measures against infectious diseases.
Notification to the public health center is completed by entering the notification of the outbreak of the new coronavirus infection into HER-SYS at the medical institution.
Regarding user registration, please apply after understanding the purpose of this system.

Usage application procedure

Please read the "Information Security Guide" below and fill out the "HER-SYS ID Registration Application Form".
When applying, please contact the Ota Ward Public Health Center.
【Notes】
・The maximum number of user IDs that can be issued for outpatient medical institutions is 6 per medical institution.
・This system contains a large amount of information about the privacy of the infected person. Please be careful about user ID management.
・After obtaining a user ID, in principle, please submit an outbreak report to HER-SYS. (Except when it is difficult to use the system due to communication conditions.)

Please submit an outbreak report to the Ota Ward Public Health Center.
