We subsidize part of the cost of the newborn hearing test so that all babies can receive it with peace of mind. In order to receive the subsidy, you will need a "newborn hearing test card" issued by the city at the time of pregnancy notification.

About newborn hearing examination consultation vote

  • Eligibility: Newborns born on or after April 1, 2019 of mothers residing in Shinjuku Ward
  • How to use: Use the medical examination slip to receive a medical examination at a contracted medical institution in Tokyo.
  • Subsidy amount: 3,000 yen (examination costs vary depending on the medical institution)
  • Expiration date: The consultation slip is valid until the 50th day after birth.

You cannot use the consultation slip issued at your previous address.
We will replace the consultation vote issued at the previous address and issue it.
You can continue to use the consultation slip issued at your previous address.

*The following documents are required to replace the consultation slip.
・Maternal and Child Health Handbook
・Documents that can be used to verify your identity
・Consultation slip issued at the previous address
*In principle, it cannot be reissued if it is lost. However, in case of unforeseen circumstances such as theft or fire, we will respond individually, so please contact us.
*If it is damaged, we will give you a new consultation slip in exchange for the damaged consultation slip.

For those who have a newborn hearing test outside of Tokyo due to returning home, etc.

for childbirth, Part of the cost is subsidized for those who cannot use the Newborn Hearing Examination Consultation Form and have had the examination at their own expense due to reasons such as having returned to their hometown outside of Tokyo. Please check here for details.

About the health guidance slip

We issue a health guidance slip that allows newborns in ward tax-exempt households and households on public assistance to receive a newborn hearing test at a hospital free of charge. For details, please contact the Health Center.
