Vaccination tickets have been sent to all eligible participants.
*Those who are 5 years old can be vaccinated with the Covid-19 vaccination ticket for infants that has already been sent.
Booster vaccination (3rd time)
We will send the vaccination ticket so that it will arrive approximately four and a half months after the second vaccination.

If you have moved to Koto Ward or have not received your vaccination ticket, please contact the Koto Ward Vaccination Call Center (0120-115-721/8:30-17:30, also available on weekends and holidays).

*For vaccinations of children, since vaccination history is managed in the maternal and child health handbook, please bring the maternal and child health handbook as much as possible on the day of vaccination.
About preliminary examination slip
- You will need to fill out a pre-screening form for vaccinations.
- On the day of the vaccination, please fill out the preliminary examination sheet using the one provided at the vaccination venue (group vaccination venue, hospital, clinic).
*For those who wish to obtain it in advance, it is distributed at the mass vaccination venue for adults (sports center in the city) and the vaccination room on the 8th floor of the Koto Ward Office.
