What is the Medical Care System for Retirees?

This system is a system in which part of the medical expenses of applicable persons is covered by contributions from the insurance system in which active salaried workers and business owners participate. Same as regular subscribers.

The retiree medical care system was abolished due to the medical system reform in 2008, and the transitional period until the end of 2014 has ended, so there will be no new application from 2015 onwards. However, those who became eligible for the medical care system for retirees by the end of March 2015 will be covered by this system until they reach the age of 65.

Eligible people

Persons who meet all of the following requirements are eligible for the retiree medical care system.

  1. Those under the age of 65 who are enrolled in National Health Insurance
  2. Those who are receiving an old-age (retirement) pension from the Employees' Pension or Mutual Aid Pension, and have been enrolled in these pension systems for 20 years or more, or have been enrolled in these pension systems for 10 years or more after the age of 40.


In addition, those who meet all of the following requirements are eligible to become dependents of the Retiree Medical Care System.

  1. Those under the age of 65 who are enrolled in National Health Insurance
  2. Those who are dependent on a retired insured person and whose annual income is less than 1.3 million yen (1.8 million yen for those aged 60 and over and those with disabilities)
