What you need for your application

2. Family register (within 1 month of issuance, if the family register of the applicant and the child are different, one copy of each is required)
   After April 1, 2012, we can issue copy of family register to acquire to apply for special child rearing allowance free in Katsushika City. When applying for issuance of a family register, please offer that it is for special child rearing allowance application.
3. Prescribed medical certificate or physical disability certificate, copy of love notebook
   The medical certificate is available at the counter below. In addition, depending on the disability, it may be possible to omit the medical certificate from the certificate of the physically disabled or the certificate of love, so please contact us for details.
4. deposit passbook
   If you are using a bank that does not issue a deposit passbook, please contact us. 
5. For confirmation of your Individual Number, a copy of your Individual Number notification card or a copy of your resident card with your Individual Number written on it. You can confirm your personal number and identity with a single card. Please contact us for more information.

In the case of procedure, copy of special child rearing allowance certificate is necessary. Please note that although you are required to submit a report of current status in August every year, it will take about three months to receive a new certificate. If you do not have it, please contact the Child Allowance Section.
