How to Use

1 Application for use

After the 25th week of pregnancy, the user himself/herself should come to the counter of the health center where the application was made. You will receive a usage approval notice from the city.

• Maternal and Child Health Handbook
• 1 identity verification document with a photo issued by a public office (driver's license, My Number card, passport, residence card, etc.)
• If the application is made by an agent, a power of attorney and identification documents of the agent are required.

• Power of Attorney (PDF 24.7KB)

• Inhabitant tax tax exemption certificate for all households (Inhabitant tax exempt households who wish to reduce the user fee. However, those who have filed a tax return in Katsushika City can omit it.)
• Public assistance receipt certificate (for those who are households on public assistance and wish to be exempted from the user's contribution)

2 (visit)

If you wish to visit the facility, please contact the facility in advance by phone and bring the usage approval notice and maternal and child health handbook.

3 Reservation

Please call the facility in advance and let them know when you will be using it.

4 Usage confirmation

After childbirth, we will call the facility to confirm the date of use.
