If a person with a physical disability is recognized as having a disability stipulated in the Law for the Welfare of Persons with Physical Disabilities, it is issued based on the application of the person (or guardian if under the age of 15).
This notebook is necessary to receive various types of assistance.
Grades from 1 to 7 are certified according to the degree of disability. (However, a certificate will not be issued for grade 7 physical disabilities only.)
If you want to know your grade to be certified
The degree of disability is diagnosed by a designated doctor and certified by a Tokyo metropolitan judge.
If you want to know the grade before applying for the notebook, please consult with a designated doctor. However, please note that the grade may differ from the diagnosis at the certification stage of the Tokyo Metropolitan Government.
In addition to grades, notebooks are classified by type ("Type 1" and "Type 2").
Please note that the service content may differ depending on the type.
Application procedure
Bring the following items and apply at the welfare office in charge.
- 1 face photo (length 4 cm x width 3 cm, no hat, no mask, upper body taken within 1 year)
- Medical certificate/opinion for the physically disabled by a designated doctor (doctor stipulated by the Law for the Welfare of the Physically Disabled) (the designated form is available at each welfare office.・You can download it from the “Written Opinion” page.)

Card-style physical disability certificate
In addition to conventional paper format, card format can be selected.
Features of the card format
- Card size (length 53.98 mm, width 85.6 mm)
- Photos are printed in black and white (the size is slightly smaller)
- Plastic card (no IC chip installed)
- Use as a set with a separate paper volume that describes various service information
- Compared to the paper format, it may take longer to issue.
Application method
- New applicants can choose when applying.
- If you wish to exchange, please bring a new face photo (4 cm in height, 3 cm in width, no hat, no mask, upper body taken within 1 year) and a physical disability certificate, and apply for reissuance. Please go.
From application to delivery of the notebook
Handing over of the notebook will be about one and a half to two months after the application.
