Inoculation ticket to be used for the 1st and 2nd inoculations
A vaccination ticket will be sent around the end of the month before the child turns 6 months old. (Those born before July 14, 2022)
*Those born on or after July 15, 2022 will not be able to complete the first vaccination (up to the third dose) by March 31, 2023 (Friday). For those who have completed the second vaccination, it will be sent around the middle of the month following the month in which the second vaccination was given.

About preliminary examination slip
- You will need to fill out a pre-screening form for vaccinations.
