We hand maternal and child health handbook to person (Katsushika ward resident) with resident registration in Katsushika City.

The Maternal and Child Health Handbook is a record of mother's pregnancy test results, child's growth and vaccinations. Be sure to bring it with you when you have a health checkup for pregnant women, a health checkup for your child, or vaccinations.

Keep it in a safe place as you may need it when you go on to higher education or find a job.

Get a pregnancy diagnosis at a medical institution

Please follow the procedures for issuing the Maternal and Child Health Handbook after receiving a pregnancy diagnosis at a medical institution. You will be asked to fill out a pregnancy notification form (name of the medical institution where you were diagnosed, expected date of delivery, etc.) and we will issue it. At that time, we will also give you a "pregnant woman medical checkup consultation slip".
