Target audience

  • People aged 12 and over who have a resident card in the ward and have completed the first vaccination (1st and 2nd doses).
  • People who have never been vaccinated against the Omicron strain.
  • Those who have not been vaccinated with the Takeda vaccine (Novavacs) for the 3rd to 5th doses since November 8, 2022.

About coupons (vaccination tickets)

A coupon (vaccination ticket) will be sent to those who are eligible for vaccination against the Omicron strain. If you have an unused coupon (vaccination ticket) sent by Shibuya City in the past, you can use it again this time if the description of the last vaccination is correct. At that time, please discard the coupon (inoculation ticket) of the person who does not use it.

Application for issuance of coupon (inoculation ticket)

Person who needs issuance application

  • Among the eligible people, those who moved to Shibuya Ward after being vaccinated against the new coronavirus last time
  • Those who fall under any of the following
    • Persons whose description on the coupon (vaccination ticket) has changed (address, name, etc.)
    • A person who is not listed on a resident card, etc.
    • Victims of domestic violence, stalking, etc.
    • Loss, loss, damage, etc. of coupons (vaccination tickets)

(Note) If you have moved to Shibuya Ward and have already been vaccinated against the Omicron strain vaccine (BA.1 compatible type or BA.4-5 compatible type) in the municipality before moving in, you can get a coupon ( Vaccination ticket) cannot be issued. Vaccine for Omicron strain is given once per person.

Required documents

  1. Additional vaccination coupon (vaccination ticket) issuance application form
  2. A copy of the identity verification document (driver's license, health insurance, etc.) of the person to be vaccinated
  3. A copy of the vaccination record (either of the certificate of vaccination, the vaccination record book, or the vaccination certificate)

Application method

Call center, electronic application, and mail will be accepted. You can also pick up the application form at the 7th floor of Shibuya Ward Office, Community Health Division New Corona Vaccine Counter, Branch Office, and Citizens Service Center.

(Note) The call center only accepts applications by the applicant himself/herself, and cannot accept applications by proxy.
(Note) If the application is made after the coupon (inoculation ticket) is issued, it will take about 2 weeks from the application. If you apply before issuance, we will send it according to the issue date, but if you apply just before the date of issue, it may not be in time due to the procedure.
(Note) Even those who are not listed on their resident card, or who have been victims of domestic violence or stalking, may be required to wait a few days for a new number to be issued.
(Note) Please note that applications are not accepted at branch offices or city resident service centers.
(Note) When applying to the call center, we may not be able to accept the application if the data of the vaccination record in Shibuya City cannot be confirmed among those who used the Shibuya City coupon (vaccination ticket) at the time of the first vaccination. . Application for vaccination outside the address

Even if you applied for vaccination outside your address in the past when you were vaccinated against the new coronavirus, you will need to reapply for vaccination outside your address. Please submit a copy (in writing) of the vaccination notification and the coupon (inoculation ticket) to the municipality of the place where you wish to be vaccinated.

(Note) If the city determines that maintaining and building the vaccination system is difficult, the application may be suspended by issuing the certificate of completion of vaccination notification outside the address.
(Note) You do not need to apply for out-of-address vaccination if you receive the new coronavirus vaccine at work.

About vaccines

We use a bivalent vaccine that corresponds to the Omicron strain (BA.1 type or BA4-5 type) and conventional strains. For more information, please see the page of explanation materials for vaccines for Omicron strains .

(Note) Even if the 3rd and 4th doses are not currently administered, future vaccinations will be bivalent vaccines for the Omicron strain. As a general rule, monovalent vaccines for conventional strains cannot be administered.

Difference between BA.1 compatible type and BA.4-5 compatible type

Bivalent vaccines for Omicron strains, whether BA.1 or BA.4-5, are more effective than conventional monovalent vaccines and may be effective against future mutant strains. sexuality is expected. Both BA.1 and BA.4-5 are Omicron strain types (substrains). It is suggested that the difference between BA.1 and BA.4-5 in terms of antigenicity (property that provokes an immune response) is not as large as that between the Omicron strain and the conventional strain. Number of inoculations

Vaccines for the Omicron strain (BA.1 or BA.4-5) are currently administered once per person.

Concurrent vaccination with other vaccines

Simultaneous administration of Omicron strain vaccine and other vaccines is not allowed. Be sure to inoculate at least 13 days after inoculating one. However, simultaneous vaccination is possible only for influenza vaccine. About extension of the expiration date of the new corona vaccine

The expiration date of a vaccine is set based on data collected by the company that manufactures and sells the vaccine, as to whether the quality is maintained when the vaccine is stored for a certain period of time, and is set through regulatory procedures. For this reason, even after the expiration date has been set once, if data can be collected on how the quality of the vaccine can be maintained when the vaccine is stored for a longer period of time, based on that data, it will be approved through regulatory procedures. The expiration date may be extended.

Pfizer vaccine expiry date extension

As of December 2022, the expiration date of Pfizer's vaccine for Omicron strains has been extended from 12 months to 18 months due to a notification from the government. 1. Sending coupons (vaccination tickets)

The city will send you an envelope containing a coupon for the new coronavirus vaccine (vaccination ticket).

3. Vaccination appointment

An e-mail address is required to make a reservation. However, some people need to register from My Page due to a system malfunction. Please register.
(Note) If a person who received the pediatric vaccine in the first vaccination and is over 12 years old at the time of the third vaccination, the vaccination for those aged 12 and over (Omicron strain vaccine) will be applied.

    4. Filling in the preliminary examination form

    A vaccination form must be completed prior to vaccination. Please consult your family doctor before completing the form, if necessary. Please do not use the attached PDF file for reference, as it is only a sample.

    If you wish to receive the vaccination at home

    In spite of wanting to be vaccinated for reasons such as home care and medical treatment in the city, it is difficult to vaccinate at the group vaccination venue or individual medical institution, and the vaccination is received by a family doctor's house call.     Obtaining consent for vaccination

    Vaccines will only be administered with the consent of the vaccinated person.
    (Note) If you are currently undergoing treatment for some kind of illness or if you have any concerns about getting vaccinated due to your physical condition, please consult your family doctor in advance and consider getting vaccinated.

    Precautions after inoculation

    Vaccines are expected to prevent the onset of COVID-19, but the extent to which they protect others from infecting others is not yet known. Also, at the stage where vaccination is gradually progressing, many people will not be able to receive the vaccine immediately, and both those who have been vaccinated and those who have not will have to live a social life together. . Even after vaccination, please continue to cooperate in preventive measures against infectious diseases, such as avoiding crowds, close contact, and airtightness, wearing a mask, washing hands with soap and disinfecting with alcohol for hand disinfection.
