The Japan Medical Association was selected by the Ministry of Health, Labor and Welfare as a training promotion organization for home care using ICT in 2022, and will hold a "workshop for nurses who support remote death diagnosis by doctors". It was become a thing.
In the "Implementation Plan for Regulatory Reform" (Cabinet decision on June 2, 2016), the handling of death diagnoses using information and communication equipment (ICT) is to respond to the situation where calm end-of-life care at home is difficult. , Even if a patient under his/her care dies 24 hours after the consultation, if all of the following requirements a to e are met, the doctor may conduct a face-to-face postmortem examination. It is now possible to perform a death diagnosis and issue a death certificate.
In response to this, research was conducted on the basic concept and specific procedures for performing death diagnosis using information and communication equipment (ICT) in the 2016 Health and Labor Sciences Research. (hereinafter referred to as "this guideline") for death diagnosis using (ICT).

a It is predicted that the patient will die sooner or later based on the course of face-to-face medical care by a doctor.
b There is sufficient cooperation between doctors and nurses, such as prior arrangements for handling at the end of life, and there is consent from the patient and family.
c. Despite efforts to coordinate among doctors and between medical institutions and nursing care facilities, it is difficult for doctors to conduct prompt face-to-face post-mortem examinations.
d Nurses who have received a certain amount of education in forensic medicine, etc., can promptly report information necessary for a doctor's judgment, such as matters decided in advance with a doctor, including confirmation of the three signs of death.
e Doctors who receive reports from nurses can confirm the fact of death and determine that there are no abnormalities by ascertaining the patient's situation by combining communication means using ICT such as videophone equipment.

Recently, the Japan Medical Association, which was selected by the Ministry of Health, Labor and Welfare as an organization to implement training for "certain education on forensic medicine," which is required by these guidelines for nurses who support remote diagnosis of death by doctors. As a "training session for nurses who support remote death diagnosis by doctors", it will be held as follows.
In addition, classroom lectures and group exercises are also available for doctors who may perform death diagnosis remotely with the support of the nurses who attend this workshop.

A certificate of completion will be issued to those who have completed all of the above trainings 1 to 3.

Target audience

◆ In principle, a nurse at a home-visit nursing office that meets all of the following (a), (b), (c), (d), (e), and (f)
(However, even if (b) is not satisfied, we may accept the course, so please write down the situation on the back side of the application form.)

(a) Have 5 years or more of practical experience as a nurse, and have witnessed 3 or more deaths of patients during that time.
(b) Of the practical experience as a nurse, have more than 3 years of practical experience in home-visit nursing or nursing care insurance facilities, etc., and have provided terminal care to 5 patients during that time (*1).

*1 “Performed terminal care” here means that, in the case of home-visit nursing, home-visit nursing was performed at least twice on the date of death of the patient and within 14 days before death. and the case where terminal care is provided after explaining to the family etc. In nursing-care insurance facilities, etc., cases where nurses are involved in formulating a plan for terminal care for the target resident based on the guidelines for end-of-life care of the facility, etc., and provide terminal care based on the plan say.

(c) Explain the training program to doctors with whom you are collaborating using ICT in your practice, and obtain their consent.
(d) Understand the "Guidelines for death diagnosis using information and communication equipment (ICT)" .
(e) Using a tablet, smartphone, etc. for work at the affiliated facility.
(f) After completing e-learning via the Internet and group exercises at one of the two venues, you must be able to complete the practical training (*2) by the end of March 2023. (* The deadline may be changed based on the future infection situation etc.)

*2 “On-the-job training” here refers to the content learned in lectures on forensic medicine, such as the three signs of death and rigor mortis, through participation in autopsies and autopsy observations at university forensic medicine departments and medical examination institutions. Refers to training that actually observes.
