About medical certificate

○ After September 26, 2022, among those who have been diagnosed positive, those who are 65 years old or older, those who require hospitalization, those who are pregnant, those who are at risk of aggravation, Notifications will be submitted from medical institutions only for those who need corona therapeutic drugs or oxygen administration. For those who do not fall under any of the above, the public health center cannot issue a certificate of medical treatment because they do not submit a report even if they receive a diagnosis.

○ For patients who have been diagnosed as positive for COVID-19 before September 25, 2022 , continue to use the "New Coronavirus Infected Person Information Grasp and Management Support System (HER-SYS)" provided by the Ministry of Health, Labor and Welfare. You can display the certificate of medical treatment for the new coronavirus infection by yourself from the My HER-SYS screen of ".

If the above medical certificate cannot be used , the following alternative documents may be available at insurance companies, etc. Please check with your contracted insurance company. (The public health center cannot answer this question.)

・Documents that show the results of PCR tests and antigen tests conducted at medical institutions, etc. ・Medical statement (medical management fee for hospitalization for type 2 infectious disease patients (includes temporary handling for outpatient treatment and medical fees) is stated)
・Prescription and dosing instructions describing the drug for treating COVID-19 ・Receipt results from the health follow-up center established by the local government (SMS, LINE, etc.)
・A copy of the email exchanged between the public health center and the person who tested positive
・Test results from test centers (other than medical institutions) that conduct PCR tests and antigen tests (excluding commercially available test kits)

* "Suspected disease patients (deemed positive)" (those who have not been tested at a medical institution and have been diagnosed as positive) cannot display.

Contents described in the medical treatment certificate of My HER-SYS

"Name, date of birth, HER-SYS ID, disease name, date of diagnosis, public health center in charge" will be listed.

If you have been admitted to an accommodation facility, please contact the Tokyo Metropolitan Government Infectious Disease Control Department / Accommodations Section (03-5320-4478), and if you have been hospitalized, please contact a medical institution.

After the medical treatment is canceled, we cannot provide proof of medical treatment due to aftereffects. In addition, period when we can prove becomes from day when public health center accepted report from medical institution.

In the past, Kita Ward used to handle work restriction notices with the end date of home medical treatment as home medical treatment certificates, but from March 2022, the format has been changed.
