○ If the above medical certificate cannot be used , the following alternative documents may be available at insurance companies, etc. Please check with your contracted insurance company. (The public health center cannot answer this question.)

・Documents that show the results of PCR tests and antigen tests conducted at medical institutions, etc. ・Medical statement (medical management fee for hospitalization for type 2 infectious disease patients (includes temporary handling for outpatient treatment and medical fees) is stated)
