Those who can receive medical examination
Those who apply to the following 1 to 3 are eligible for the examination.
- All people aged 70 and 75 living in Nerima Ward. Please see the target age table below.
- Not diagnosed with dementia by the date of screening.
- Even if the score on the "Checklist for self-awareness of dementia" is 20 points or more or the score is 19 points or less, there is a concern about forgetting things.

You can get tested at any medical institution. Please select the desired medical institution and apply directly.

Self-awareness checklist for dementia
This checklist was developed by the Tokyo Metropolitan Government so that you can check yourself for any decline in cognitive function.
Anyone can do it, even if they are not eligible for the checkup.
This is an A4 size sheet format file. (PDF: 695KB)
You can also do it on the Internet. Points are calculated automatically.
Flow of examination
A cherry-colored envelope containing a consultation ticket and checklist will be mailed to all eligible age groups on Monday, April 25.
- I'm going to try the "checklist for self-awareness of dementia".
- Even if your score is 20 or more or 19 or less, you can take a medical examination if you are not worried about forgetting things.
- Select a medical institution from the list in the envelope and make a reservation directly by phone.
- Fill in the medical questionnaire. You can leave blank if you do not understand.
- Don't forget to bring your ticket with you on the day.

The results of the examination will be sent to Nerima Ward from the medical institution through the Nerima Ward Medical Association.
Depending on the results, the Regional Comprehensive Support Center will ask about the situation afterwards.
