When we cannot use consultation vote of Katsushika City when we receive pregnant woman medical checkup and newborn baby hearing examination at medical institution outside Tokyo or maternity hospital, we support a part of medical examination expense.
Target audience
Those who correspond to all of the following
1 Those who have a resident registration in Katsushika City on the day of the consultation
2 Those who have undergone prenatal health checkups, prenatal ultrasound examinations, prenatal cervical cancer screenings, and newborn hearing tests at their own expense outside of contracted medical institutions in Tokyo. For midwives, those who have undergone the second and subsequent prenatal health checkups and newborn hearing tests at their own expense.
(Both are limited to Japan. Newborn hearing tests at midwives are Consultation after October 1, 2022 If you do, you will be eligible for assistance. )
3 Infants who are eligible for the newborn hearing test are those who are registered as residents in Katsushika City, and who have undergone the test up to the 50th day after birth (up to 50 days counting from the day of birth as the 0th day).
Details of subsidy
We will subsidize the number of unused prenatal health checkup consultation slips, as well as unused prenatal ultrasound examination slips, pregnant woman cervical cancer checkup slips, and newborn hearing test slips.
Prenatal health checkups (1st time), prenatal ultrasound examinations, and cervical cancer screenings for pregnant women are not covered.

Please obtain a set of application documents by one of the following methods.

2 Pick up at the Maternal and Child Health Section of the Children's Center (in Health Plaza Katsushika).

How to apply for a grant
After giving birth within a year Please apply at the Children's Center.

The certificate must be filled in by the medical institution, etc., and the certificate fee will be borne by the applicant. -

When sending by mail, please make a copy of (1) the cover page, (2) progress during pregnancy (pregnancy checkup record), and (3) newborn hearing test record. (In the case of the Mother and Child Health Handbook issued by Katsushika Ward, (2) is 8-9 pages (3) is 17 pages) -
