What is the Health and Welfare Handbook for the Mentally Disabled?

A person with a mental disability proves that they have a certain disability, and it is issued upon application.
By having this notebook, you can receive various support, so it will help people with mental disabilities to live independently and participate in society.

Target audience

Those who have a handicap in daily life and social life due to some kind of mental illness are eligible.
Note: All mental illnesses are covered, including:

  • schizophrenia
  • Depression, mood disorders such as depression
  • epilepsy
  • Drug or alcohol addiction, etc.
  • higher brain dysfunction
  • Developmental disorders (autism, Asperger's syndrome, learning disabilities, attention deficit/hyperactivity disorder, etc.)
  • Other psychiatric disorders (stress-related disorders, etc.)

Note: However, if you have an intellectual disability and do not have any of the above mental illnesses, you are not eligible for this notebook because of the Ai no Techo system.
Note: In addition, in order to receive the notebook, it is necessary that at least 6 months have passed since the first medical examination due to the mental illness.

The degree of disability ranges from grade 1 to grade 3, depending on the degree of limitation in daily and social life.

Application procedure

Documents required for application

  • Prescribed application form (available at the application window)
  • A copy of a medical certificate for the notebook (specified one) or a disability pension certificate for which mental disability is the reason for payment
  • Photograph (4 cm long x 3 cm wide, taken within the past year with no hats and showing the upper body)
  • A copy of your current notebook (only for renewal)

The application window is the Health and Welfare Center in your area.

Expiration Date/Issuance and Renewal Procedures

The certificate is valid for 2 years. expiration date 3 months ago from You can update.
Note: Until the issuance of the notebook , it can take 2 to 3 months.

Note: Those who request it will be notified by postcard that the notebook has been completed.
Note: The pick-up location will be the Health and Welfare Center window where you applied.
Note: If the application is not applicable as a result of examination by the Tokyo Metropolitan Government, a notification of disapproval will be issued.
Please update as soon as possible.

Procedures for changes, etc.

If there is a change in your address, name, degree of disability, etc., please bring your notebook and report it to the Health and Welfare Center in your area.
Note: If the notebook is lost or damaged, please apply for reissuance.

With the start of the My Number system, from January 2016, it will be necessary to enter your My Number when applying for the Mental Disability Health and Welfare Notebook.
During the procedure, we will check your personal number and identify yourself with your personal number card, etc. Thank you for your cooperation.

*If you are a person with a mental disability, you can receive medical expenses subsidies for outpatient psychiatric hospital visits through medical care for independence support (outpatient mental hospital) and disability welfare services under the Act on Comprehensive Support for Persons with Disabilities regardless of the presence or absence of a certificate.
