* For those who have not received the 3rd or 4th vaccination and who have an unused 3rd or 4th vaccination ticket, the city will not send the ticket, so advance designation will not be made again. You must make an appointment for vaccination yourself.

● Those who wish to be vaccinated under the conditions specified in advance

In order to reduce the burden of making appointments for vaccinations, for those aged 65 and over, the date and time of vaccination and the venue designated by the ward are printed on the vaccination ticket.

Those who receive the vaccination under the conditions designated by the ward do not need to make a reservation for the vaccination themselves, as the quota has already been secured.

How to change/cancel pre-specified conditions

If you wish to receive the Omicron strain bivalent vaccine at a time other than the date and venue designated by the city, the following procedures are required. The Community Activity Center will assist you in canceling or rebooking between 9:00 am and 5:00 pm on weekdays.

  • If you wish to change the date and time of inoculation or the venue

Please change the vaccination date and time specified on the call center (0120-168-185) or the reservation site.

  • If you wish to have an individual vaccination at a designated medical institution in the city
    1. Please make a reservation directly with each designated medical institution.
    2. Please cancel the conditions specified on the call center (0120-168-185) or the reservation site.
  • Other than the above, if any of the following applies to you, please cancel from the call center (0120-168-185) or the reservation site.
  1. Those who do not wish to receive the Omicron strain bivalent vaccine
  2. Those who receive Omicron strain bivalent vaccine at a large-scale vaccination venue in Tokyo or at a facility for the elderly
