[Reissue] If you lose your vaccination ticket (coupon) or screening ticket
Apply online or by mail. Vaccination tickets for the 1st and 2nd doses, child vaccinations (1st and 2nd doses), and infant vaccinations (1st to 3rd doses) can be reissued at the counter of a nearby health center.
When applying online
Please access the input form for the “New Corona Vaccination Voucher Issuance Application Form” and enter the necessary information to apply.
When applying by mail
Please mail the vaccination ticket issuance application form to Suginami Public Health Center Health Prevention Division New Coronavirus Vaccination Section (5-20-1 Ogikubo, Suginami-ku, Tokyo 167-0051). On the front of the envelope, please write “Inoculation ticket issuance application”.
If the applicant and the recipient are not the same person, please enclose a copy of the applicant's identity verification documents and a power of attorney (with the signature and seal of the person who requires certification). If the applicant is an adult guardian or curator, please enclose a copy of the certificate of registered information.

- If your address is written on the back of your identity verification document, please submit a copy of the back as well.
- Please enclose only a copy of the front side of your My Number Card, and do not enclose a copy of the back side.
- My number notification card cannot be used as an identification document.

Inoculation outside the address (when a person who has a residence record other than Suginami Ward wants to be vaccinated in Suginami Ward)
As a general rule, inoculations are to be carried out in the municipality where the resident card is located. However, if for some unavoidable reason it is difficult to receive the vaccination at the location on the resident record, it is possible to receive the vaccination by notifying the municipality where the vaccination will be performed in advance.
Even if you have a residence record outside of Suginami Ward and submitted a notification of vaccination outside of your address to Suginami Ward at the time of the previous vaccination , you will need to submit the notification again when you receive the 3rd to 5th vaccinations.
A copy of the application form "Out-of-Address Vaccination Notification (New Coronavirus Infectious Disease)" and a copy of the vaccination ticket (coupon) issued at the resident registration area. Enclose a copy of what has been given, a self-addressed envelope with an 84-yen stamp, and write on the surface of the envelope, "Address out-of-town vaccination notification" , Suginami Public Health Center Health Prevention Division New Coronavirus Vaccination Section ( 167-0051 5-20-1 Ogikubo, Suginami-ku).

(Note) For those who have evacuated to Suginami Ward due to the Great East Japan Earthquake and have submitted a notification of evacuation, a "vaccination ticket (coupon)" and "address out-of-town vaccination notification certificate" will be issued from the municipality where you evacuated from with your resident card. (There is no need to submit a “notification of vaccination outside the address” to Suginami Ward.) Please use the vaccination ticket (coupon) you received from your former municipality to make a reservation at Suginami Ward during the appropriate period.
Procedures when moving to Suginami Ward
If you have moved to Suginami Ward and wish to receive one of the 1st to 5th vaccinations, the 1st to 3rd childhood vaccinations, or the 1st to 3rd infant vaccinations, you will be required to renew your residence in Suginami even if the vaccination ticket was issued at your previous address. It is necessary to issue an inoculation ticket of the ward.
In addition, even if the previous vaccination was given in the municipality before moving in, the next vaccination ticket will not be automatically sent, so you will need to apply for issuance of the vaccination ticket.
(Example) If you received your 4th vaccination on August 1st as a resident of Nakano Ward and moved to Suginami Ward on August 10th, please apply to Suginami Ward for issuance of a vaccination ticket via the Internet or by mail.
(Note) Vaccination tickets issued at the previous address cannot be used.
