With revision of law about the prevention of infectious disease and medical care for patient of infectious disease, office work such as designation of tuberculosis medical institution was transferred from Tokyo on April 1, 2012. The place to apply is the public health center that has jurisdiction over the medical institution.
1. When applying for a new designated medical institution

*Retroactive may not be possible. -
In the case of reapplication after withdrawal, it will be a non-designated medical institution until the date of redesignation is decided.
If the public-funded patient is undergoing medical treatment, please submit a "retroactive request" together so that the designated date will continue.

3. When there is a change in the specified contents
If there is a change in the name or location of the medical institution that has already been designated, you will need to submit a notification of the change. Please note that the procedure differs depending on the content of the change.
1. If you decline the current designation and apply for a new designation
- When the founder changes (e.g. parent → child, change of legal entity, etc.)
- When the founder changes from an individual to a corporation (including so-called "one-person medical corporations") or from a corporation to an individual.
- When relocating a medical institution (including temporary relocation due to extension or renovation)
- When changing a clinic to a hospital or a hospital to a clinic

- When simply changing the name of the medical institution.
- When there is a change in the name of the location name of the medical institution and the lot number due to a change in the residential address, etc.
