Target audience
◆40 to 74 years old Those who are enrolled in the Shinjuku City National Health Insurance or are receiving public assistance, etc.
◆ 75 years old or older Those who are enrolled in the Tokyo Medical Care System for the Advanced Elderly or are receiving public assistance, etc.
* Age is the full age at the time of the birthday by March 31, 2023
*Health checkups for those between the ages of 40 and 74 who are enrolled in health insurance other than the Shinjuku City National Health Insurance (including dependents) are conducted by each medical insurer. For details, please contact your medical insurer.
* Health checkup forms were sent out in late April 2029 for those aged 75 and over, and in late May 2029 for those aged 74 and under.

About provision of specific health checkup information by online qualification confirmation system

When providing this information, if the subscriber does not wish to provide Shinjuku City with information on specific health examinations conducted by the former insurer via the online qualification confirmation system, the subscriber will notify Shinjuku City of that fact.
