About change accompanied with limitation of outbreak report of new coronavirus infectious disease


On September 12, 2022, the Ministry of Health, Labor and Welfare announced that the number of people who should report the outbreak of new coronavirus infectious diseases will be limited. Regarding the issuance of medical certificate documents for those who have been diagnosed by a medical institution after September 26, 2022, the following measures will be taken.

[Persons subject to reporting]

(1) Persons aged 65 or older, (2) Persons who require hospitalization, (3) Persons who are at risk of becoming severe and need to be administered novel coronavirus therapeutic drugs, or persons who are at risk of becoming severe and who are currently at risk of becoming severely affected by COVID-19. For those who need new oxygen administration due to illness and (4) pregnant women, My HER-SYS medical certificate or "medical information statement" can be issued as before.

For those other than the above, we cannot issue a medical certificate for My HER-SYS or a "medical information statement", so please use alternative documents such as result notification emails from the positive person registration center.

Regarding medical care certificates issued (displayed) from My HER-SYS

From April 27, 2020, a function has been added to My HER-SYS that allows the positive person to display and prove the date of diagnosis by their own operation, so please use it.
If the period of accommodation or home medical treatment is within the period (e.g., 7 days regardless of whether symptoms are present or asymptomatic) in accordance with the standards for canceling medical treatment by the Ministry of Health, Labor and Welfare, the Life Insurance Association or The Ministry of Health, Labor and Welfare indicates that it can be used to claim insurance claims from insurance companies affiliated with the General Insurance Association of Japan.
Currently, due to the rapid increase in the number of positive cases, it takes about one month from the time an application is submitted for the medical care information statement issued by the public health center to be sent. We recommend that you use

*Those who can display a medical certificate on My HER-SYS are those who have undergone tests (PCR test/antigen test) and have been diagnosed with a new coronavirus infection by a doctor. If you are a "special suspected patient (deemed positive)", you cannot display the medical treatment certificate on My HER-SYS, so please follow the instructions in "Regarding the medical information form" below to the public health center. Please apply for issuance by mail.
* My HER-SYS will be able to issue a certificate after the medical treatment period is over. The issue button cannot be pressed during the medical treatment period.

My HER-SYS

My HER-SYS is a health management system of the Ministry of Health, Labor and Welfare that allows people who test positive to enter their own health status and that of their family members using a smartphone or computer.
The URL and HER-SYS ID required to register for My HER-SYS have been sent by short message to the phone number of each positive person reported by the medical institution.
*Short messages will not be sent if the phone number reported by the medical institution is a phone number that cannot receive short messages, such as landlines. If you would like a HER-SYS ID, please contact the Public Health Center (03-5984-4671).

How to view the medical certificate on My HER-SYS

You can display the medical certificate by the following operation.
For information on how to submit a certificate of medical treatment, please check with the place where the certificate of medical treatment is submitted.
* Currently, there is no print function on the medical treatment certificate screen of My HER-SYS. If you want to print it, use the screenshot (screen save) function of your smartphone, save it once, and then print it.

If My HER-SYS medical certificate cannot be used

About the Medical Information Statement

As mentioned above, you may be able to prove your medical treatment yourself using the My HER-SYS screen, so please consider this.
For those who cannot prove medical treatment using My HER-SYS, or because the medical treatment period exceeds the period according to the medical treatment cancellation criteria of the Ministry of Health, Labor and Welfare (e.g., 7 days regardless of whether symptoms are present or asymptomatic), My HER-SYS For those who cannot use the medical certificate of (My Hersys), it is issued by mail application only once per person, regardless of the course of medical treatment.
Depending on the epidemic situation, it may take a while (about 1 to 2 months) to be issued.

Contents of medical care information statement

・ Date of first medical examination (date of first visit to a medical institution for the novel coronavirus infection)
・ Diagnosis date (date of diagnosis of new coronavirus infection)
・ Date of onset (date when symptoms of new coronavirus infection appeared. Sample collection date for those who are asymptomatic)
・Medical institution report date (the date the medical institution reported that the patient was diagnosed with the new coronavirus infection)
・ Treatment end date * The treatment end date is the day when the discharge criteria for the new coronavirus infection (based on the notification of the Ministry of Health, Labor and Welfare) are met. Hospitalizations for other illnesses and voluntary isolation periods are not covered.

The medical treatment period indicated by the Ministry of Health, Labor and Welfare is as follows.

For those with symptoms, in principle 7 days from the date of onset and up to 24 hours after symptoms have subsided

For asymptomatic persons, in principle, 7 days from the date of sample collection.If symptoms appear during treatment, the treatment period for "symptomatic persons" will start from that day.

If it takes more than 8 days from the date of onset (the date of sample collection for asymptomatic patients) to the end of medical treatment, please enclose a “statement of reasons for extension” describing the details of your health observation and the reason for the extension. Please check the contents and note that depending on the symptoms, etc., the treatment period may not be as described.

Submitted materials cannot be returned. Please be sure to send a copy.

About hospitalization recommendation (including extension) and public expense decision notice

We will send a hospitalization recommendation (including extension) and public expense decision notice to those who have been hospitalized for medical treatment of the new coronavirus infection. No application is required.
Shipping may be delayed depending on the infection status.
Public medical expenses are covered by the hospitalization medical expenses for the recommended hospitalization period until it is judged that there is no infectiousness according to the discharge criteria based on the notice of the Ministry of Health, Labor and Welfare. Hospitalization after being confirmed as non-infectious, and expenses other than inpatient medical expenses (pajamas rental, outpatient medical expenses after completion of medical treatment, etc.) are not covered.
A copy of the Public Expenditure Decision Notification will also be sent from the Public Health Center to the inpatient medical institution.

About accommodation certificate

For those who received medical treatment at an accommodation facility, the medical treatment certificate from MYHER-SYS can be substituted as long as it is within the period (7 days regardless of whether symptoms are present or asymptomatic) in accordance with the standards for canceling medical treatment by the Ministry of Health, Labor and Welfare. About certificates for patients with suspected special cases (deemed positive)

From July 14, 2020, if a medical institution reports that a close contact (with a clear history of contact with an infected person), such as a family member of a positive person, has become symptomatic, a doctor will It is possible to make a diagnosis and report to the public health center based on "clinical symptoms" without conducting a PCR test, etc.
It is possible to issue a "Medical Care Information Statement" even for patients who have not been tested and have submitted an "occurrence report" from a medical institution.
