What you need for vaccination (what to bring)

  1. Inoculation ticket sent from Koto Ward
  2. Preliminary examination sheet (Please fill in in advance by the day of vaccination.)
  3. Identity verification documents (driver's license, health insurance card, etc. that shows your name, date of birth, and address)
