The Medical Care System for the Advanced Elderly is a medical system for those aged 75 and over and those aged 65 and over with certain disabilities. It started in April 2008 to clarify the burdens of the working generation and the elderly generation, to make the system fair and easy to understand, and to make the medical system sustainable into the future.
If you become eligible for the system, you will be removed from the medical insurance (national health insurance, social insurance, etc.) that you were enrolled in until then, and you will become an insured person of the medical system for the elderly aged 75 or over.

Persons with certain disabilities are those who:
- Part of Physically Disabled Person's Handbook Grade 1 to Grade 3 and Grade 4 (Note 1)
- Mental Disability Health and Welfare Notebook Level 1 and 2
- Ai no Techo (Rehabilitation Techo) 1st and 2nd
- National Pension Certificate (Disability Pension Grade 1 and 2)
Note 1: A part of the physical disability certificate 4th grade is as follows.

About health insurance for spouses aged 74 or under who newly enrolled in the Medical Care System for the Advanced Elderly
In some cases, it may be necessary to switch (enroll) health insurance for spouses, etc.
If your spouse is enrolled in National Health Insurance
No procedure is required. You will continue to be insured by the National Health Insurance.
If the spouse is a dependent of the company's health insurance (social insurance, etc.)
It is necessary to complete the procedures for switching (subscribing) to health insurance .

About moving procedures
If you change your address, please notify the Residents' Office or the National Health Insurance and Pension Section, Senior Citizens Qualification Section (City Office Main Building 2nd Floor, "Late-stage Senior Citizens Medical Care System Counter") within 14 days.
* If you have applied for a change of address at the ward office, you do not need to go through the procedure at the medical care system counter for the elderly aged 75 or over.
