By presenting the "certificate of medical treatment for specific diseases" at the window of the medical institution, the maximum copayment for specific diseases will be 10,000 yen per month per medical institution.
If you have newly enrolled in the Medical Care System for the Advanced Elderly (75 years old, recognized as disabled) or have moved from outside of Tokyo, and you have a Certificate of Treatment for Specific Diseases, you will need to apply for continuation.

About reissuance of specific illness medical treatment certificate

If you have lost, defaced, or damaged your Certificate of Medical Treatment for Specific Diseases, please apply for reissuance.
In principle, the new Certificate of Medical Treatment for Specific Diseases will be mailed (ordinary mail) to the address on your resident card in about one week from the date of notification.
