We perform 2021 medical checkup for elderly aged 75 or over.

Medical examinations for the elderly aged 75 or over are comprehensively evaluated based on the characteristics of the elderly, such as maintenance and promotion of health, prevention of aggravation of lifestyle-related diseases, maintenance and assurance of quality of life (QOL), and frailty. This is done for the purpose of understanding

Target audience

Those who fall under any of the following

  1. Those aged 75 or older (born before June 1, 1947) who are insured under the Medical Care System for the Advanced Elderly in Itabashi Ward
  2. Those aged 65 to 74 who are insured under the medical system for the elderly aged 75 or over in Itabashi Ward and who have been recognized as having a certain degree of disability.
  3. Those who were born between June 1, 1947 and October 31, 1947 and are switching to the Medical Care System for the Advanced Elderly from other than Itabashi Ward National Health Insurance

Note: Those who fall under any of the following are not eligible for medical checkups for the latter-stage elderly.

  • a.Those who have been hospitalized in a hospital or clinic continuously for 6 months or more
  • B. Those who are in the following facilities
    • Support facility for people with disabilities
    • Special elderly nursing home
    • Designated Nursing Care Welfare Facility
    • Nursing care facility for the elderly
    • Designated long-term care medical facility
    • Among serviced housing for the elderly, facilities equivalent to fee-based nursing homes
    • Paid Nursing Home (Note)
    • Low cost nursing home (Note)
    • Nursing home for the elderly specified by the Elderly Welfare Law (Note)

  • C. Those who are eligible for health checkups conducted by their employer

How to see a doctor

"Late-stage elderly medical examination ticket" When "Health Insurance Card (Late-stage Elderly Medical Care Insurance Card)" Please bring it with you to the doctor.
Note: Please see the attached file "List of medical institutions" and "Corrections to the list of medical institutions" for information on non-consultation days and appointments. We will update the errata list of implementation medical institutions as needed.
Note: It is expected that it will be very crowded as the examination deadline approaches. Please see a doctor as soon as possible.

Note: For those who have moved in during the consultation period, we will send out a consultation ticket at any time. For details, please contact the person in charge below.
Note: For those who fall under "Target 3" above, we will send you a "Late-stage elderly medical checkup ticket" at the end of the month before your birth month. If you were born on or after November 1st, we will send you a ticket for general health checkup before the start of the health checkup period.
Note: Please see the attached file "Sample Ticket" (the upper left margin is the address column. The central part is the consultation ticket (divided into 6)). From 2013, it will be a sticker format, and for those who are eligible, we will print and send a checkup ticket for colorectal cancer, hepatitis virus, and prostate cancer that can be examined at the same time. When you visit the clinic, please do not remove the ticket from the cardboard and bring it with you. In addition, only two tickets for medical checkups for the latter-stage elderly are printed, but you can only have one checkup.
Note: Please note that even if you receive a consultation ticket, those who do not fall under the above "eligible persons" will not be able to have a consultation.

[Additional inspection items]
In Itabashi Ward, in addition to the [basic inspection items], everyone can take the following inspection items.
However, some or all may be omitted at the discretion of the doctor.

Explanation of results

Please be sure to receive an explanation of the medical examination results from the doctor who consulted you by the end of the month following the examination date.

Response to the self-medication tax system

Please keep the results of each medical checkup in a safe place, as they will be required when applying for the self-medication tax system (special exemption for medical expense deductions). For the self-medication tax system, please check the website of the National Tax Agency or the Ministry of Health, Labor and Welfare (see link below).

Regarding information inquiries and provision between insurers of copies of records of specific health checkups and specific health guidance

On February 20, 2021, a ministerial ordinance was enacted to partially revise the standards for implementing specific health checkups and specific health guidance. It is no longer necessary to obtain the consent of the individual when taking over a copy of the If you do not wish to provide it, you can make a request from the subscriber.
Itabashi Ward does not currently require the former insurer to provide a copy of records such as medical examination results, so there is no need to make a request to Itabashi Ward. If you are switching from Itabashi Ward, you will need to apply to the new insurer. Please contact your new insurer for details.

Those who have applied to use their My Number Card as a health insurance card will be able to view information such as health checkups. (For details, see the Mynaportal website.)
