- AstraZeneca's booster vaccination (3rd vaccination) has not been approved under the Pharmaceutical Affairs Law in Japan.
- If you have received one dose of the Janssen Pharma vaccine overseas, it is considered that you have completed the second dose in Japan. Also, if you have been vaccinated twice overseas, it is considered that you have completed the third vaccination in Japan.

- For those who used the Suginami Ward vaccination ticket for the 3rd time, a ticket for the 4th vaccination will be sent the month before the month in which the vaccination is available (there is no need to apply for the 4th vaccination ticket again at Suginami Ward). ).
- If you have moved out of the ward after receiving the first and second vaccinations in Suginami Ward, you will need to apply for a ticket for the third vaccination to the municipality where you moved to. For procedures, please contact the local government where you are moving to.
- There are currently no scientific findings, such as the safety, of alternate vaccination of domestically approved vaccines (Pfizer, Moderna, AstraZeneca, Novavax, Janssen Pharma) and other vaccines.

When applying by mail
Please mail the required documents to Suginami Public Health Center Health Prevention Division New Coronavirus Vaccination Section (5-20-1 Ogikubo, Suginami-ku, Tokyo 167-0051). On the front of the envelope, please write “Inoculation ticket issuance application”.
