If you have soiled or lost your National Health Insurance card or National Health Insurance Elderly Beneficiary Certificate , please apply for reissuance.

  • If you are 75 years old or older, please see the Medical Care System for the Advanced Elderly page.
  • If you lose it, you cannot stop using the National Health Insurance card or change the code number. We recommend that you report the loss or theft to the police station (police box).

Temporary operation to prevent the spread of new coronavirus infections

Households with unpaid National Health Insurance premiums past the deadline for payment will also receive applications by mail as a temporary operation. ( Two of the following) National health insurance card, health insurance, seamen's insurance, medical insurance for the elderly or long-term care insurance, national pension handbook, member of the National Public Servants Mutual Aid Association or the Local Public Servants Mutual Aid Association maternity health handbook, certificate of loss of eligibility for health insurance, welfare certificate, health insurance day laborer special insurance certificate, basic resident register card without photo, student ID card without photo, various medical beneficiary certificates, etc.

  • My number (personal number) confirmation documents for the head of the household

"My number (individual number) confirmation documents" are my number cards (individual number cards), notification cards, etc.
